In kidney surgery, bleeding is one of the most important issues. In partial nephrectomy, as a "blood stopper", we used surgycell and ankaferd which is used traditionally in Turkish medicine. ⋯ Ankaferd is a substance which can be used effectively for controlling acute bleeding in kidney surgery (Tab. 2, Ref. 19). The tuberculin skin test (TST) has recently been proposed as a screening procedure for latent TB prior to anti-tumor necrosis factor (TNF) alpha therapy. Our aim was to evaluate TST levels in patients receiving anti TNF alpha due to ankylosing spondylitis (AS) and rheumatoid arthritis (RA). ⋯ This is the first study to evaluate the correlation between the use of multiple immunosuppressive agents and TST. We determined that TST is correlated with disease duration in AS and with the use of multiple immunosuppressive agents in RA (Tab. 3, Ref. 21).

Hydatid disease is a parasitic tapeworm infestation. Anaphylactic reactions as a result of cyst perforation generally occur after trauma during a spread of cystic fluid in the intravascular space, which may also initiate anaphylaxis. The reported case was a 17-year-old boy who was admitted to the hospital emergency department with a sudden onset of nausea, vomiting and fainting, resulting in death. ⋯ Autopsy macroscopically revealed hydatid cysts in the liver. In the histopathological examination, hydatid cyst was recognized by scolices and also an inflammatory infiltration composed mainly of mast cells in the myocardium was detected. Sudden death in this case was attributed to allergic myocardial infarction due to intravascular spread of the hydatid cyst content (Fig. 2, Ref. 10).

Complicated appendectomy in children in relation to laparoscopic vs open procedures.
The role of laparoscopic appendectomy in the management of complicated appendicitis remains unsettled with reports of a higher incidence of postoperative intraperitoneal abscess. Most studies on laparoscopic appendectomy in children have been done in the Western population. This retrospective review has been done to compare laparoscopic appendectomy with open appendectomy in children with complicated appendicitis in Chittagong Medical Hospital, Bangladesh. ⋯ This review has confirmed that the longer the operating time, the lower the incidence of wound infection and shorter the postoperative stay for laparoscopic appendectomy. We examined the expression pattern of smooth muscle actin (SMA), h-caldesmon (HCD), calponin (CALP), placental alkaline phosphatase (PLAP) and human placental lactogen (HPL) in benign and malignant spindle cell superficial soft tissue tumors in order to determine the role of these markers in differential diagnosis. Archival tissue from 38 patients with superficial smooth muscle cell and so-called fibrohistiocytic tumors (8 benign fibrous histiocytomas (BFHs), 6 dermatofibrosarcoma protuberans (DFPT), 9 malignant fibrous histiocytomas (MFHs), 9 leiomyomas (LMs) and 6 leiomyosarcomas (LMSs)) were immunostained with antibodies against SMA, HCD, CALP, PLAP and HPL. smooth muscle cell (SMC) tumors showed significantly high immunopositivity for HCD than that of so-called fibrohistiocytic tumors (p is less than or equal to 0.05) but 1/3 of DFPT and MFH cases and half of BFH cases also showed HCD immunopositivity; thus, this difference is debatable and not highly discriminative as expected. ⋯ Superficial soft tissue tumors did not express c-kit. In conclusion, HCD and PLAP can be used as ancillary immunomarkers in differential diagnosis of SMC tumors (Tab. 2, Fig. 7, Ref. 37).
