Tension pneumopericardium is an uncommon complication of mechanical ventilaton. It may indeed be life-threatening for hemodynamic compromise and circulatory collapse. ⋯ Although timely rescue measures were performed, the patient died finally. We report the case to help us to be aware of and take precautions against this fatal condition during intrahospital patient transportation.

To determine the efficacy of Sustained Natural Apophyseal Glides (SNAGs) with and without Isometric Exercise Training Program (IETP) in Non-specific Neck Pain (NSNP) Methods: This randomized control trial of one year duration was conducted at out-patient department of Physiotherapy and Rehabilitation, Khyber Teaching Hospital (KTH) Peshawar, Pakistan from July 2012 to June 2013. The sample of 102 patients of NSNP were randomly selected through simple random sampling technique, and placed into two groups. The SNAGs manual physical therapy technique with IETP was applied on 51 patients in group A and SNAGs manual physical therapy techniques was applied alone on 51 patients in group B. The duration of intervention was 6 weeks, at 4 times per week. The Neck Disability Index (NDI) and Visual Analog Scale (VAS) for neck pain were assessment tools used for all patients before and after 6 weeks of physical therapy intervention. All the patients were assessed through NDI and VAS before intervention and at the completion of 6 weeks program. The data of all 102 was analyzed by SPSS-20 and statistical test was applied at 95% level of significance determine the efficacy of both the treatments interventions and compare with each other. ⋯ Patients with non-specific neck pain treated with SNAGs manual physical therapy techniques and followed by IETP was more effective in reduction of pain and enhancement of function, as compared to those patients treated with SNAGs manual physical therapy techniques alone.

Objective : To summarize the functional outcome of tarsometatarsal joint fracture-dislocation managed according to Myerson classification. Methods : Total eighty cases of tarsometatarsal joint fracture-dislocation were treated from Mar 2004 to Feb 2012. According to the Myerson classification, there were 14 cases in type A, 12 cases in type B1, 28 cases in type B2, 11 cases in type C1 and 15 cases in type C2. ⋯ In type B, single or double incisions with screw or plate fixation is enough, while in type A and type C, double or triple incisions with screw or plate fixation in medial joints and Kirschner wire fixation in lateral joints are needed. Postoperatively, the type B patients had better prognosis than type A and type C patients. However, the concomitant injuries around the tarsometatarsal joint were not included in Myerson classification, which is the limitation but cannot be neglected.

This study was conducted to determine the common mutation of low density lipoprotein receptor in patients with familial hypercholesterolemia (FH) in our population and identify the different point mutation in the LDL-receptor gene. The main aim of this study was to reduce the cost of PCR without extracting DNA and do the diagnosis at single step. ⋯ Screening method based on PCR by using Kappa direct PCR could be a faster and cheaper method with least contamination for screening a large number of FH patients for mutation of LDLR gene.

Background and objective : During the past few years, Computer-based assessment (CBA) has gained popularkity as a testing modality. This assessment offers several advantages over paper based assessment (PBA) testing. The objective of this study was to find out residents' perception of this method of assessment. Methods : The post graduate residents of Dow University of Health Sciences in the field of Surgery, Medicine, Gynecology and Obstetrics experienced their first formative Computer-based assessment (CBA) in year 2013.Immediately after formative CBA, an anonymous paper based questionnaire was distributed amongst the residents and response was sought for their self-perceived computer usage competence before starting residency, perceptions regarding CBA method and to determine their preference for PBA or CBA in future assessment preferences. ⋯ Resident's perception for CBA is good and they recommend its use in future assessment as well. However, to take maximal advantage of this technology, faculty should be trained to develop questions not only with text and pictures but with audio and video support.
