A tracheostomy is an artificial opening in the trachea that is held open by a tracheostomy tube. Inspired and expired air bypasses the mouth and nose. There is a range of reasons for performing a tracheostomy, including long-term mechanical respiratory support from, for example, a ventilator.

Breathlessness is a common symptom associated with a variety of disorders. It is defined as 'an abnormal awareness or difficulty with breathing' (Bourke and Brewis, 1998). ⋯ It may also be the presenting symptom of life-threatening disease. Chronic breathlessness develops over several months or years.

This study examined decontamination and infection control procedures in general practice. Four primary care trusts participated with a response rate of 80 per cent. The study showed evidence of poor compliance with national standards and examples of unsafe practice.
