A 30-year-old male fire eater presented following aspiration of hydrocarbon fuel during a performance. A plain chest radiograph carried out 2 h after aspiration showed left lower lobe consolidation. ⋯ The clinical course was complicated by the development of large pleural effusions, pneumatocoeles and a spontaneous pneumothorax. Early abnormalities on a plain chest radiograph following suspected hydrocarbon aspiration require close monitoring for the development of further life-threatening complications.

Case ReportsOesophageal and tracheal perforation in thyroid B-cell lymphoma.
We report an unusual case of spontaneous oesophageal and tracheal perforation secondary to thyroid B-cell lymphoma in a previously healthy 76-year-old woman. Findings on chest radiography, CT and oesophagram are correlated to endoscopy, surgery and pathology. Such perforation is a rare and serious complication that should be specifically considered when a patient with a neck neoplasm develops acute respiratory or gastrointestinal symptoms.

Radical radiotherapy with high-dose-rate brachytherapy for uterine cervix cancer long-term results.
The aim of this is to report the results of radical radiotherapy in carcinoma of the cervix treated by high-dose rate (HDR) intracavitary brachytherapy and external beam radiotherapy (XRT) at a single centre in Singapore. This is a retrospective analysis of 106 consecutive cases with histologically proven cervical cancer, treated by HDR brachytherapy and XRT at the Mount Elizabeth Hospital from 1990 to 1993. External beam radiotherapy to the pelvis was delivered with 6 MV photons, to 45-50.4 Gy in 1.8 Gy fractions. ⋯ Late complications were mild (Radiation Therapy Oncology Group score 1-2), except for one patient with grade 4 rectal toxicity. The complication rates were 8, 14 and 45%, respectively, for the rectum, bladder and vagina (stenosis). The use of two to three fractions of HDR intracavitary brachytherapy in addition to pelvic XRT achieves good outcomes.

Preparing to sit the Royal Australia and New Zealand College of Radiologists Faculty of Radiation Oncology Fellowship Part 2 examination: the value of a workshop including practice and feedback.
A workshop has been held annually to help prepare candidates to sit the Royal Australia and New Zealand College of Radiologists Part 2 Faculty of Radiation Oncology examination. This study examined the value of such a course and its component parts and assessed attendees' learning environments. We collected detailed information from participants before and after the training workshop in 2005. ⋯ The course design described seems reasonably satisfactory in that it included the components ranked most highly by candidates. We also identified a number of variations that may be useful for future workshops. Although learning environments were generally good, we identified a perceived problem with workloads affecting a significant number of registrars.

Comparative StudyComparison of CT and positron emission tomography/CT coregistered images in planning radical radiotherapy in patients with non-small-cell lung cancer.
Imaging with F-18 fluorodeoxyglucose positron emission tomography (PET) significantly improves lung cancer staging, especially when PET and CT information are combined. We describe a method for obtaining CT and PET images at separate acquisitions, which allows coregistration and incorporation of PET information into the radiotherapy (RT) planning process for non-small-cell lung cancer. The influence of PET information on RT planning was analysed for 10 consecutive patients. ⋯ In four patients, three with atelectasis, there were significant reductions in V20 (percentage of the total lung volume receiving 20 Gy or more). Use of coregistered PET/CT images significantly altered treatment plans in a majority of cases. This method could be used in routine practice at centres without access to a combined PET/CT scanner .
