Randomized Controlled Trial Comparative Study Clinical TrialInteradductor approach to obturator nerve blockade for spastic conditions of adductor thigh muscles.
The authors describe a new approach for the management of adductor muscle spasm associated with multiple sclerosis and paraplegia. ⋯ The interadductor approach is a new approach based on the anatomy of the obturator nerve trunk, which, though in the obturator canal, is shielded by its osseous part from the anteroposterior perspective of the traditional approach. The interadductor approach allows needle positioning inside the obturator canal through a mediolateral perspective, thus facilitating the blockade of the obturator nerve trunk before it branches immediately outside the canal. The new approach proved to be successful, reproducible and without complications.

Randomized Controlled Trial Clinical TrialEfficacy of stellate ganglion block: a clinical study with bupivacaine.
When administering stellate ganglion blocks (e.g., to pain patients), it may be essential to know whether the sympathetic block is complete. The aim of the present study was to study the efficacy of stellate ganglion blocks using different concentrations and volumes of local anesthetic and different sites of injection. ⋯ It was difficult to achieve a block that met all five established criteria. When assessing the efficacy of a stellate ganglion block, it is essential to evaluate the effects on vasoconstrictor and sudomotor fibers.

Randomized Controlled Trial Comparative Study Clinical TrialPatient-controlled analgesia with epidural meperidine after elective cesarean section.
We report the first controlled analysis of the use of patient-controlled epidural meperidine. This randomized, prospective study compares the efficacy and safety of patient-controlled epidural meperidine to conventional intramuscular meperidine for the management of postoperative pain after elective cesarean delivery. ⋯ Patient-controlled epidural meperidine after cesarean delivery more effectively manages postoperative pain than conventional intramuscular use. The technique is preferred by both patients and nursing staff and can be used in the ward setting with appropriate organization and education. Respiratory depression, if it occurs, should present early after epidural bolus administration.

Skin blood flow changes after intradermal injection of ropivacaine in various concentrations with or without epinephrine were investigated using laser Doppler flowmetry. ⋯ The combination of ropivacaine and adrenaline did not accentuate but instead diminished the vasoconstrictive effect of epinephrine.

Randomized Controlled Trial Comparative Study Clinical TrialTechnical and light microscopic comparison of four different small-diameter catheters used for continuous spinal anesthesia.
Recently several manufacturers developed ultra-thin catheters for continuous spinal anesthesia. Their use may be associated with technical problems. ⋯ The use of the 32-gauge polyurethane catheter was associated with problems that limit its clinical usefulness. Nylon and polyimide catheters with stainless steel stylets were more resistant to stretching. The nylon catheter was most resistant to adhesion of fibrinous material.
