Case Reports[A case of recurrent gastrointestinal stromal tumor with complete response from treatment with reduced dose of imatinib mesylate].
We report a patient who had a complete response by treatment with 200 mg of imatinib mesylate daily for peritoneal recurrences of gastrointestinal stromal tumor(GIST)of the stomach. On March 2007, a 68-year-old woman underwent distal gastrectomy for GIST of the stomach. ⋯ After reduction of imatinib mesylate, the adverse reactions resolved. Peritoneal dissemination disappeared on CT scan from April 2010, and complete response has been maintained for 18 months.

Case Reports[A case of severe subcutaneous emphysema during laparoscopic right-hemicolectomy].
A 76-year-old woman, weighing 40 kg, was scheduled for laparoscopic right-hemicolectomy. Forty minutes after starting the laparoscopic procedure, PaCO2 increased to 64.3 mmHg. ⋯ In the laparoscopic procedure for the treatment of colectomy, it is important to discover abnormalities such as snowball crepitation and arrhythmia, and to monitor ETCO2 continuously in order to avoid massive subcutaneous emphysema and hypercapnia. During a laparoscopic procedure, hypercapnia might cause acidosis, arrhythmia, hypotension, myocardial ischemia, and cardiac arrest.

[Prevention of nausea and vomiting in pain medication for digestive cancer].
Nausea and vomiting are common side effects due to opioid therapy, and may greatly impede the quality of life of cancer patients. A preventive method for nausea and vomiting has not yet been established. We developed a clinical pathway(CP) for cancer pain management in which prochlorperazine is used for the prevention of nausea and vomiting caused by opioids. ⋯ The incidence of nausea and vomiting of those patients was 15. 8% which was lower than the results of other previous clinical trials. However, we could not show the effectiveness of prochlorperazine. Prochlorperazine, which is a dopamine D2 receptor antagonist, may show limited utility for the prevention of nausea and vomiting; however, in opioid therapy, histamine receptor(H1)prevention is also important.

[Hyperglycemia due to atypical antipsychotics in cancer patients with delirium].
We investigated hyperglycemia due to atypical antipsychotics in cancer patients with delirium. Using patient records retrospectively, we investigated 189 cancer patients who were diagnosed with delirium between June 2008 and May 2009. ⋯ Three percent(5/154)of them were admitted with over 350 mg/dL blood glucose during their atypical antipsychotics treatment, however, all cases did not relate the adverse reaction of hyperglycemia with the use of antipsychotics. Most cancer patients with delirium used atypical antipsychotics, and the incidence of hyperglycemia was low.
