Carcinomas of unknown primary are cancers that are pathologically diagnosed as carcinomas based on the findings of metastatic lesion biopsy but in which the primary site remains unknown even after thorough and detailed examinations. In the clinical setting, pathological diagnosis and diagnostic imaging are very important for cases of carcinoma of unknown primary. ⋯ In clinical practice, it is very important not to miss cases of favorable prognosis group, which is 20% of unknown cancer. Recently, microarray studies have been performed for cases of carcinoma of unknown primary.

Review Case Reports[Prolonged disturbance of consciousness and respiratory depression induced by controlled-release morphine, requiring long-term naloxone administration in a hemodialysis patient with cancer--a case report].
An 82-year-old female on hemodialysis was diagnosed with advanced breast cancer. She received 20 mg of controlled-release oral morphine per day for the relief of cancer pain. After a total dose of 30 mg was administered, she was hospitalized with disturbance of consciousness. ⋯ Morphine is therefore not recommended for use in renal failure and dialysis patients. The use of fentanyl or oxycodone is recommended as an alternative opioid. It is essential that medical staff are aware that these patients have an increased risk of developing serious morphine-related toxicity.

Review[Radiological evaluation of pancreatic ductal adenocarcinoma].
Curative resection is one of the key treatments that influences the survival of pancreatic ductal adenocarcinoma patients. For curative resection, and to avoid unnecessary resection, it is important to diagnose and find the stage of pancreatic carcinoma correctly. ⋯ In this article, we reviewed the effective CTprotocols for detecting pancreatic carcinoma and the CTimaging findings of pancreatic carcinoma that will effect the resectability of the tumor. We also reviewed advantages and disadvantages of other imaging modalities, and imaging findings of some pancreatic carcinomas other than ductal adenocarcinoma.

Review[Prostatectomy-pros and cons on open surgery/laparoscopic surgery/robot-assisted surgery].
We have 3 options when perfoming prostatectomy for the treatment of localized prostate cancer. Those are retropubic radical prostatectomy, laparoscopic radical prostatectomy and robot-assisted laparoscopic radical prostatectomy. ⋯ However, it would be very difficult to adopt it in Japan because it would pose economical difficulties. The administrative assistance in the insurance systems requireds much more than we have.
