Case Reports[Shiga toxin-associated hemolytic uremic syndrome with hypocomplementemia. Report of one case].
The Shiga toxin associated (Stx) hemolytic uremic syndrome (HUS) is an important cause of acute renal failure (ARF) and the most common cause of thrombotic microangiopathy (TMA) in pediatrics. Primary atypical HUS (aHUS) is a rare disease due to a genetic defect in the alternative complement pathway. Both diseases may share clinical and laboratory elements, making differential diagnosis difficult, such as the presence of diarrhea in aHUS or complement alterations in HUS-Stx. ⋯ Alterations in complement factors were observed. Finally, the diagnosis of HUS-Stx was established. The patient evolved favorably with recovery of renal function.

[Medical teleconsultation. Analysis and recommendations from the ethics department of the Chilean medical association].
The possibility of allowing patients access to health professionals, has been greatly facilitated by advances in technology. Indeed, nowadays it is possible not only direct contact between one health professional with another, but also the possibility of sending images and other tests to consult distant colleagues. This has undoubtedly enabled better health care for many patients. ⋯ It is this last way of relationship, which has already spread as a practice in normal times, which arouses apprehensions about the ethical requirements that a consultation must meet. This work by the Ethics Department of the Chilean Medical Association seeks to reflect on the ethical demands of a medical consultation and on the shortcomings that teleconsultation has. It also aims to propose several recommendations, so that this new form of doctor-patient relationship serves as a complement to traditional care, without jeopardizing the objectives of a medical action.

[The possible poisons contained in the arrow that killed Ferdinand Magellan].
Ferdinand Magellan's maritime expedition that resulted in the circumnavigation of the Earth and the discovery of the strait that bears his name is among the greatest feats in history. The trip, which took more than three years, was not completed by Magellan, who died on the island of Mactan, Philippines in a scuffle with the locals. As reported in Magellan's voyage journal written by Pigafetta, Magellan died after receiving a poisoned arrow in his right leg. ⋯ These poisons are mainly derived from Aconitum and other species, such as Strychnos, Lophopetalum, Beaumontia, and Strophanthus. They contain cardiac alkaloids and glycosides, which can produce neurological and cardiac effects in just a few minutes. We argue that these toxic effects hindered the withdrawal of Magellan from the beach, facilitating his death in hands of the locals.
