Data on the clinical characteristics of delayed treatment initiation among pulmonary tuberculosis (TB) patients are lacking. Thus, this study aimed to identify the factors associated with delayed treatment in culture-confirmed pulmonary TB and to assess outcomes of delayed treatment. ⋯ Given that delayed treatment is associated with worse outcomes in South Korea, targeted interventions to increase awareness on TB in the healthcare community are necessary for additional mycobacterial tests and consults of suspicious patients to TB specialists.

The emerging infection of the 2019 novel coronavirus (2019-nCoV) in late December, 2019 in Wuhan, China, has caused an extreme health concern, with many patients having progressed to acute respiratory disease or other complications in a short period. Meanwhile, the risk factors associated with the disease progression still remain elusive. ⋯ A higher cell count of total lymphocytes may indicate a better outcome of the disease, and immune response may be a vital factor for directing disease progression in the early stage of 2019-nCoV infection.

The risk of blood loss differs among subtypes of revision total hip arthroplasty (THA), and different tranexamic acid (TXA) protocols have rarely been studied in those conditions. The present study aimed to evaluate the efficacy and safety of intravenous and intravenous plus topical TXA in a subtype of revision THA. ⋯ Both intravenous TXA and combined TXA significantly reduced perioperative transfusion volume and calculated blood loss in unilateral major revision THA with comparable perioperative transfusion rate and risk of VTE. More researches are required to explore the optimal TXA administration protocol in subtypes of revision THA.

The mortality of acute respiratory distress syndrome (ARDS) remains high, and mechanical ventilation (MV) is an essential means of treatment. During MV, people realize the benefits of spontaneous breathing and the disadvantages of uncontrolled spontaneous breathing. Current methods of monitoring spontaneous breathing include oesophageal manometry, P0.1, and diaphragm function monitoring. ⋯ The driving pressure is a new indicator that reflects the strain of the lung, which indicates the volumetric injury of the lung and is independently associated with mortality in ARDS patients. Moreover, in recent studies, driving pressure monitoring has been shown to be feasible in assisted support ventilation. This review summarizes the current state of spontaneous breathing and examines whether it is convenient to monitor driving pressure during spontaneous breathing to achieve lung protection ventilation.

A randomized, double-blind, placebo-controlled study evaluating the efficacy of combination olanzapine, ondansetron and dexamethasone for prevention of chemotherapy-induced nausea and vomiting in patients receiving doxorubicin plus cyclophosphamide.
Since most of Thai cancer patients receiving high emetogenic chemotherapy do not have access to neurokinin-1 (NK-1) receptor antagonists or palonosetron as recommended by international guidelines for chemotherapy-induced nausea and vomiting (CINV) prevention. We decided to evaluate the efficacy of olanzapine with the real-life practice antiemetic drugs ondansetron and dexamethasone, in prevention of CINV resulting from doxorubicin plus cyclophosphamide regimen in early-stage breast cancer patients. ⋯ Olanzapine 10 mg combined with ondansetron and dexamethasone was more effective than a placebo in preventing CINV resulting from doxorubicin plus cyclophosphamide in early-stage breast cancer patients, especially in the first 24 hours after chemotherapy administration. The short duration of olanzapine was safe and well tolerated.
