Randomized Controlled Trial Clinical Trial[Randomized controlled trial on effectiveness of the community re-entry program to inpatients with schizophrenia spectrum disorder, centering around acquisition of illness self-management knowledge].
The objective of this study was to examine by randomized controlled trial the effectiveness and safety of the Japanese version of the Community Re-Entry Program, a manualized psycho-educational program with cognitive-behavioral therapy techniques, in discharge preparation for inpatients with schizophrenia. Our hypotheses are that firstly the program is effective for patients in acquiring illness self-management knowledge, that secondly the program shows positive effects on objective behavior and symptoms, and that thirdly the program has no adverse effects on subjective quality of life (QOL). ⋯ The Japanese version of the program was effective and relatively safe for schizophrenia spectrum disorder inpatients for learning illness self-management knowledge in preparation for discharge and planning community re-entry.

ReviewConsultation-liaison psychiatry: an international perspective.
Consultation-liaison (C-L) psychiatry is the specialty of physical/psychiatric comorbidity and somatization. As such, it addresses the commonest forms of psychiatric presentation in the community; it is mainstream psychiatry. Its name reflects the fact that most of the patients seen by C-L psychiatrists have to be managed jointly with a non-psychiatrist doctor and other professionals. ⋯ These differences permit a refreshing opportunity to re-examine the ways in which patients with physical/psychiatric comorbidity and somatization can be helped. The International Organization for Consultation-Liaison Psychiatry was formed to facilitate this process. It has identified challenges that are universal, and which if not met rapidly and efficiently will damage our ability to provide appropriate care for the patients whom we serve.

Review[What should general hospital psychiatry do in a community?].
Some experiences in Nagano Red Cross hospital and Nagano Prefecture are presented, and the role of general hospital psychiatry (GHP) in a community is discussed. Psychiatric services in Nagano prefecture with population 2.21 million consist of four blocks. Our unit is in north block, providing treatment for acute phase and, in 2000, 1504 cases were new outpatients, daily outpatients were 198 cases and new inpatients were 604 cases including 146 emergency inpatients. ⋯ In order to promote the above it is necessary to self-evaluate GHP concerning the difference with specialized psychiatric hospitals or clinics. From these viewpoints GHP will become a core of community psychiatry. Currently, however, preparation concerned with GHP is poor compared with other advanced countries, so an aggressive improvement in medical policy is expected.

Case Reports[Perospirone therapy in elderly patients with schizophrenia].
Perospirone is a novel serotonin-2 and dopamine-2 receptor antagonist (SDA) developed in Japan. Premarketing trials suggested that this agent was effective in reducing positive and negative symptoms of schizophrenia and had a favorable side-effect profile. However, these trials included only a few elderly patients, so the usefulness of perospirone in this population remains unknown. ⋯ The clinical course of this patient suggests that the clinical characteristics of perospirone and risperidone may be different, even though these agents are categorized into the same class of antipsychotics, SDA. Because this is a case report, evaluations are limited the clinical properties of perospirone. Further examination is necessary to evaluate the efficacy and safety of perospirone for elderly patients with schizophrenia, who are more vulnerable to the side effect of antipsychotics than the younger population.

[Screening and intervention for depressive mothers of new-born infants].
BACKGROUND AND THE AIM OF THE STUDY: Postnatal depression is a key concept for mother-infant mental health. Evidence of its impact on mother-infant relationship has been increasingly demonstrated in recent years. Therefore optimal intervention is important for women and their babies' mental health. Identifying risk factors of postnatal depression and developing screening system are needed. Study I Hospital-based prospective study--onset and course of Postnatal depression and developing screening system. ⋯ The onset of postnatal depression can be within the first week after delivery. The use of the EPDS during the first week is a simple and useful screening for early onset case. Maternal complications and related medical factors might be the risk factors of mood disturbance during early postnatal period.
