Comparative StudyContinuous positive airway pressure versus continuous negative pressure around the chest for patients with acute exacerbation of chronic obstructive pulmonary disease in the intensive care unit: a pilot study.
With chronic obstructive pulmonary disease (COPD) exacerbations, continuous positive airway pressure (CPAP) has been used to overcome the threshold load provided by intrinsic positive end expiratory pressure and decrease the inspiratory work of breathing. In this pilot study, we observed whether a continuous negative pressure (CNP) around the thorax and upper abdomen with a shell and wrap would provide a similar level of relief in dyspnoea. ⋯ The CNP was similar to CPAP except CNP was more comfortable.

Initial experience of endobronchial silicon stents from a tertiary care centre in North India.
Central airway obstruction (CAO) is defined as obstruction of trachea and principal bronchi. Therapeutic rigid bronchoscopy with tracheobronchial stenting using silicon stents is a well established procedure in the management of such conditions. However, there is limited experience with this technique in India. ⋯ Rigid bronchoscopy with silicon stent placement is an effective and suitable method of relieving the distressing symptoms due to benign or malignant airway obstruction.

Cardiopulmonary exercise testing in evaluation of patients of chronic obstructive pulmonary disease.
Objective assessment of severity in patients with chronic obstructive pulmonary disease (COPD) is mainly limited to pulmonary function testing performed at rest. But, accurate assessment of exercise capacity in patients with COPD may be possible with cardiopulmonary exercise testing (CPET). ⋯ Cardiopulmonary exercise testing is useful to determine the causes of exercise limitation and to assess the maximal exercise capacity of patients with COPD.

Retrospective study of interstitial lung disease in a tertiary care centre in India.
There has been a recent surge in the recognition of interstitial lung disease (ILD) in India. ⋯ Interstitial lung diseases (ILDs) appear to be under-reported from India. Lack of recognition and inadequate availability of diagnostic facilities, like high resolution computed tomography (HRCT) of the chest may be some of the reasons for this. The diagnostic yield from TBLB in our study was high at 96 percent. The TBLB may be used as the initial, cost-effective and safe tool for confirmation of aetiological diagnosis in most patients with diffuse parenchymal lung diseases.

Case ReportsBronchoscopic management of bronchopleural fistula.
In recent years successful bronchoscopic management of bronchopleural fistulas (BPFs) by locating its site and then blocking the leaking segment with any of the several agents available has gained recognition. It is now considered as an alternate mode of management of BPF. Here we present a case of non-resolving pneumothorax that was managed successfully using bronchoscopic glue (cyanoacrylate glue) instillation.
