[Advanced non-small cell lung cancer - pioneer in personalized oncology].
In no other cancer is clinical practice as close to the ideals of personalized oncology as in the treatment of non-small cell lung cancer (NSCLC). Here, we briefly outline the basic principles of molecular analysis. The focus of the article is on the clinical utility of targeted therapy. Finally, we highlight the challenges for medical staff and structural solutions reflecting the situation in Germany.

Esophageal motor disorders are an important cause of dysphagia but can also be associated with retrosternal pain and heartburn as well as regurgitation. In extreme cases, patients are not able to eat appropriately and lose weight. Repetitive aspiration can occur and may cause pulmonological complications. ⋯ Balloon dilatation and Heller myotomy are long established and more or less equally effective therapeutic options for patients with achalasia. Peroral endoscopic myotomy (POEM) enhances the therapeutic armamentarium for achalasia and hypertensive/spastic motor disorders since 2010. For hypotensive motor disorders, which may occur as a complication of e.g., rheumatological diseases or idiopathically, therapeutic options are still limited.

Extrapulmonary tuberculosis (TB) presents unique diagnostic and therapeutic challenges. The site of involvement can vary widely, with common sites including the lymph nodes, pleura, skin, ear, nose and throat, genitourinary system, pericardium, gastrointestinal tract, bones and joints, and central nervous system. ⋯ Treatment of extrapulmonary TB generally follows similar principles to pulmonary TB, but the duration of treatment depends on the site of involvement and the extent of the disease. Increased awareness among healthcare providers is essential for the timely recognition and effective management of extrapulmonary TB cases.

[Histology-agnostic tumor treatment - a farewell to tumor entities?].
Considerable efforts concerning the molecular characterization and targeted treatment of cancer have significantly improved treatment options and prognosis of tumor patients. Nevertheless, in tumor entities without recurrent genetic alterations the application of molecular testing for potentially targetable lesions remains heterogeneous and, in most cases, the approval of targeted therapies is still restricted to defined tumor entities harboring corresponding predictive biomarkers. ⋯ In the meantime, a handful targeted therapeutics addressing NRTK and RET fusions, the BRAF V600E mutation or different features of defective DNA mismatch repair and high tumor mutational burden has been approved for histology-agnostic treatment of tumors harboring these target lesions. Ongoing molecularly stratified basket trials will further investigate the tumor-agnostic efficacy of different targeted treatment approaches.

[Cardiovascular assessment and management of patients undergoing non-cardiac surgeries - A case based resumé].
In August 2022, the European Society of Cardiology (ESC) published new guidelines on cardiovascular assessment and management of patients undergoing non-cardiac surgery. In order to cope with the complexity of the patients and the multidisciplinary, representatives of the European Society of Anesthesia and Intensive Care Medicine as well as 2 patient representatives were involved in the development of the published document in addition to authors from the European Society of Cardiology. As a result, the document contains 147 recommendations on this complex topic, with more than half of them being grade I recommendations. ⋯ Particular attention was also paid to recommendations for action for a structured preoperative risk stratification. The new guidelines also focus on increasing vigilance for perioperative myocardial damage and identifying it at an early stage. In the following article, some of the essential innovations and revisions will be highlighted using practical clinical case studies.
