Randomized Controlled Trial[Roflumilast in combination with long-acting bronchodilators in the management of patients with severe and very severe COPD. A cost-effectiveness analysis for Germany].
To calculate the cost-effectiveness of roflumilast in combination with a long-acting beta agonist (LABA) versus LABA as a monotherapy in patients with severe and very severe COPD in Germany. ⋯ The model calculation indicates that the cost-effectiveness of roflumilast as an add-on to LABA in patients with severe and very severe COPD is comparable to the cost-effectiveness of established and reimbursed treatment options in Germany. Analogue consideration of the cost-effectiveness of the treatment options LAMA, LABA and ICS are advisable.

Randomized Controlled Trial[Effects of daily physical exercise at school on cardiovascular risk--results of a 2-year cluster-randomized study].
It was the aim of this study to measure the effects over two years of daily sport activity during the school-day on their physical fitness (primary endpoint), motor coordination and blood pressure (secondary endpoints). ⋯ The results indicate that daily physical exercise during school hours should be given greater importance. But it will require a long-term trial to determine whether promotion of increased physical activity at school influences the prevalence of cardiovascular risk factors when the pupils reach adulthood.

Randomized Controlled Trial[Procaine infusion for pain treatment of acute pancreatitis: a randomized, placebo-controlled double-blind trial].
Acute pancreatitis is commonly associated with severe abdominal pain, making early pain relief a primary goal of the treatment. This study was undertaken to assess the efficacy of a continuous intravenous (i.v.) infusion of procaine compared with that of a placebo infusion in providing pain relief in patients with acute pancreatitis. ⋯ These data do not indicate a clinically meaningful analgesic effect of i.v. infusion of procaine (maximal amount. 1.92 g/24h) in patients with acute pancreatitis, but suggested that this infusion actually increased the feeling of bodily discomfort and nausea. We thus conclude that a constant i.v. infusion of procaine should no longer be recommended for pain relief in patients with acute pancreatitis anymore.

Randomized Controlled Trial Multicenter Study[Effectiveness of telephone aftercare following a smoking cessation program for women on inpatient rehabilitation].
Rehabilitation centres for mothers are a suitable setting to offer smoking cessation treatment to these women. Telephone aftercare could enhance the effectiveness of an inpatient smoking cessation program. We investigated whether non-directive, supportive aftercare or a structured, smoking specific counselling give more benefit. ⋯ A structured telephone aftercare proves to be an effective intervention for women, which stabilizes the abstinence rate following a clinic based smoking cessation program.

Randomized Controlled Trial[Effect of whole body vibration exercise on osteoporotic risk factors].
Whole body vibration (WBV) training is a new approach which is currently discussed in the context of reducing the risk of osteoporotic fractures. The study was undertaken to determine the effect of one-year WBV exercise on bone mineral density (BMD) and the number of falls. ⋯ The multifunctional training resulted in a gain of BMD at the lumbar spine. Vibration training did not enhance the effect on bone but significantly reduced falls.
