Neuropathic pain in the hands due to carpal tunnel syndrome (CTS) disturbs sleep and affects the quality of life. ⋯ We believe that US-guided partial release of the TCL using a needle can be an effective and safe technique for treating chronic refractory pain due to CTS. It can potentially be attempted before surgical treatment.

Gray Matter Abnormalities in Patients with Chronic Primary Pain: A Coordinate-Based Meta-Analysis.
Many structural magnetic resonance imaging (MRI) studies have used voxel-based morphometry (VBM) to identify gray matter abnormalities in patients with chronic primary pain (CPP), but the findings have been inconsistent. ⋯ We identified gray matter changes in CPP patients and female patients, as well as a close relationship between CPP and mental disorders. With the chronicity of pain leads to changes in relevant brain regions, which makes treatment more challenging and may have synergistic effects with affective disorders. More prospective longitudinal structural MRI studies of CPP examining the associations between those variables and gray matter in a larger population should be conducted. Additional prospective longitudinal structural MRI studies of CPP with larger sample sizes to confirm the relationships between these variables and gray matter are needed as well as gender differences of CPP in brain structure and function.

Pregnant women are among the groups most affected by the United States opioid epidemic. ⋯ Hospitalized pregnant women may be classified based on comorbid conditions into 2 latent classes ("birth complications" and "pre-existing and pregnancy-related morbidities"), with the former exhibiting greater OUD frequency than the latter. These findings can inform health care needs of populations with a high-risk for OUD.

Lumpectomy is important for preventing malignant changes in benign tumors and diagnosing malignant tumors. Intercostal nerve blocks (ICNBs) are useful for breast lumpectomy as either the primary anesthetic or as an adjuvant anesthetic procedure. To our knowledge, no studies have evaluated the association between Horner syndrome and ICNB. ⋯ Horner's syndrome, intercostal nerve block, breast lumpectomy, enhanced recovery.

Many self-report scales have been developed. Among them, are those clinically useful scales for quantifying chronic pain (such as the Numeric Rating Scale), which are useful for determining the effectiveness of treatment, and multifaceted and comprehensive pain assessment scales that are used to determine therapeutic strategies. The representative measure of the latter is the West Haven Yale Multidimensional Pain Inventory (WHY-MPI), which constitutes a system for classifying patients with chronic pain termed the multiaxial assessment of pain (MAP), proposed by Turk and Rudy. ⋯ The study findings support the applicability of the MPI-J as a clinical assessment scale in Japanese patients with chronic musculoskeletal pain.
