Ultrasound (US) has well-documented utility in critical procedures performed in the emergency department. It has been described as a "skill integral to the practice of emergency medicine" in the 2007 Model of Clinical Practice of Emergency Medicine. One of the ideal uses for US in critical care may be in the performance of emergent cricothyroidotomy. To the best of our knowledge there is currently no description of how to perform an US-guided open cricothyroidotomy in the literature. ⋯ Ultrasound-guided bougie-assisted cricothyroidotomy is a novel technique that may be beneficial in emergent open cricothyroidotomy. The data suggest that this technique is rapid, with a median time to completion of 26.2 seconds. The data also suggest that the procedure may have a low failure rate, with 20 of 21 cadavers undergoing successful cricothyroidotomy.

Long-term voice outcomes after thyroplasty for unilateral vocal fold paralysis.
To investigate the long-term clinical efficacy and stability of thyroplasty type I for unilateral vocal cord palsy, and to identify the appropriate timing of posttreatment evaluations for determination of long-term voice outcome. Study ⋯ Thyroplasty type I may provide evidence that voice outcome progressively evolves during the first years after the surgical procedure, and that subsequent vocal improvement presented long-lasting stabilization. To assess the long-term voice quality, it may be enough to perform the voice evaluation at 1 year after the procedure.

The anesthesiologist's chief responsibility is to ensure ventilation of the patient who is undergoing surgery or to apply resuscitation measures in life-threatening situations. Cricothyrotomy is one among many options available for maintaining airway patency. Usually it is the last resort but can be a lifesaving maneuver in the cannot-intubate, cannot-ventilate patient. The various types include cannula insertion, percutaneous puncture and dilatation, and surgical cricothyrotomy; techniques may be combined, as in the case of surgically inserting a cannula over a trocar.
