Motor blockade is reduced with ropivacaine 0.125% for parturient-controlled epidural analgesia during labour.
To compare the effect on the incidence of motor block by reducing the concentration of ropivacaine from 0.2% to 0.125% in parturient-controlled epidural analgesia (PCEA) for labour. ⋯ Both ropivacaine 0.2% and 0.125% provided comparably effective analgesia but motor block occurred more commonly in the 0.2% group.

Target-controlled infusions (TCI) aim to provide constant, user-defined blood concentrations of a drug. The infusion device of such a system is controlled by a microprocessor that uses population pharmacokinetic data and the individual patient's weight and age to continuously calculate the required drug infusion rate to replace losses from the blood compartment due to drug distribution and metabolism. ⋯ TCI of propofol for general anesthesia and sedation are now widely used, but the technology is also being extended to the fields of intra- and postoperative analgesia and to patient-maintained sedation and analgesia. Early work on targeting the effect site (brain) is under way and target-controlled propofol infusions are being used in experimental closed-loop anesthesia systems.

To assess the most appropriate postoperative analgesic technique after hip surgery. ⋯ After THA, i.v. PCA with morphine, continuous "3-in-1" block, and PCEA provided comparable pain relief. Because it induces the fewest technical problems and side effects, continuous "3-in-1" block is the preferred technique.

Continuous spinal anaesthesia/analgesia for abdominal aortic aneurysm repair and post-operative pain management.
The intra-operative management of two patients with chronic obstructive pulmonary disease and cardiovascular pathology, who underwent peripheral reconstructive vascular surgery under continuous spinal anaesthesia, is described. Furthermore, continuous intrathecal analgesia was also continued in the post-operative period and provided effective pain relief that was reflected by the favourable surgical outcome.
