Pioneer works on therapeutic hypothermia (TH) half a century ago already showed promising results but clinical application was limited by a lack of understanding of the underlying pathophysiology, lack of reliable method for temperature control and lack of intensive care facilities to deal with possible complications. More recently, 2 studies in 2002 supported the application of moderate TH (32.0-34.0℃) in post-cardiac arrest patients. Although the studies included only patients suffering from out-of-hospital VF, many ICUs world-wide are applying the therapy to all post-cardiac arrest patients irrespective of site or presenting rhythm. ⋯ De-cooling and rewarming is especially challenging because complications can be serious if temperature rise by more than 1℃ every 3-5 hours. Life-theatening hyperkalaemia can occur especially if patient suffers from renal insufficiency. Fever is a frequent complication either due to infection or post-cardiac arrest syndrome but patient must be kept normothermic for 72 hours.

Cardiac arrest induced by tension pneumothorax during ventilating bronchoscopy -A case report-.
Tension pneumothorax during ventilating bronchoscopy for foreign body removal is a rare but life-threatening complication. The authors present a case of cardiac arrest caused by tension pneumothorax in a 9-month-old girl who underwent ventilating bronchoscopy for foreign body (peanut) removal. ⋯ The patient was successfully resuscitated by cardiopulmonary resuscitation and chest tube insertion. The airway injury was effectively repaired by thoracotomy under extracorporeal membrane oxygenation.

The survival outcome following pediatric cardiac arrest still remains poor. Survival to hospital discharge ranges anywhere from 0 to 38% when considering both out-of-hospital and in-hospital arrests, with up to 50% of the survivors having neurologic injury. ⋯ We review the current applications of induced hypothermia in pediatric patients following cardiac arrest after searching the current literature through Pubmed and Ovid journal databases. We put forth compiled recommendations/guidelines for initiating hypothermia therapy, its maintenance, associated monitoring and suggested adjunctive therapies to produce favorable neurologic and survival outcomes.

Major advances in the field of pediatric cardiac arrest (CA) were made during the last decade, starting with the publication of pediatric Utstein guidelines, the 2005 recommendations by the International Liaison Committee on Resuscitation, and culminating in multicenter collaborations. The epidemiology and pathophysiology of in-hospital and out-of-hospital CA are now well described. ⋯ We reviewed landmark articles related to pediatric CA published during the last decade. We present the current knowledge of epidemiology, pathophysiology and treatment of CA relevant to pre-hospital and acute care health practitioners.

Prehospital pediatric cardiac arrest is a rare event compared with adult cardiac arrest. Despite the recent advancements in postresuscitation care improving the outcome of adult patients, similar evidence is lacking in pediatric victims of cardiac arrest. In this brief article, the current data on pediatric cardiac arrest occurring in the prehospital setting are reviewed. ⋯ The neurologic outcome of survivors is good in 24-31% of patients. Current evidence is insufficient to strongly support or refute the use of mild therapeutic hypothermia during the postresuscitation phase in pediatric patients. The application of a goal-directed treatment protocol for pediatric cardiac arrest and postresuscitation syndrome needs to be evaluated.
