Biodegradable fixation of mandibular fractures in children: stability and early results.
The aim of this study was to assess the safety and efficiency of biodegradable self-reinforced (SR-PLDLA) bone plates and screws in open reduction and internal fixation of mandible fractures in children. ⋯ Pediatric patients benefit from the advantages of resorbable materials, especially from faster mobilization and the avoidance of secondary removal operations. Based on these preliminary results, self-reinforced fixation devices are safe and efficient in the treatment of pediatric mandible fractures. However, further clinical investigations are necessary to evaluate the long-term reliability.

Comparative StudyRigid internal fixation with titanium versus bioresorbable miniplates in the repair of mandibular fractures in rabbits.
The purpose of this study was to compare by qualitative histology the efficacy of rigid internal fixation with titanium system and the Lacto Sorb system in mandibular fractures in rabbits. Thirty male adult rabbits Oryctolagus cuniculus were used. Unilateral mandibular osteotomies were performed between the canine and first premolar. ⋯ But after 60 days, bony healing was similar in both groups. It is concluded that both PLLA/PGA and titanium plates and screws provide sufficient strength to permit mandibular bone healing. The resorption process of PLLA/PGA osteosynthesis material did not cause acute or chronic inflammatory reaction or foreign body reaction during the studied period.

Transmasseteric antero-parotid approach for open reduction and internal fixation of condylar fractures.
The morbidity that results from surgical approaches to the condylar neck, and the time-consuming nature of the operation inhibits many surgeons from using open reduction and internal fixation for the treatment of condylar fractures. The many approaches that have been described stand testimony to the disadvantages of the individual techniques. The most common problems are limited access and injury to the facial nerve. We describe the transmasseteric antero-parotid (TMAP) technique, which offers swift access to the condylar neck while substantially reducing the risk to the facial nerve and eliminating the complications associated with transparotid approaches.

Comparative StudyComparison of the healing process in plated and non-plated fractures of the mandible in rats.
We compared the healing process of plated and non-plated fractures. The mandibles of 72 male Wistar rats were fractured and more either plated or not plated (n = 36 in each group). The healing process of the two conditions was studied histologically and immunohistochemically using a specific antibody to bone morphogenetic protein-2 (BMP-2). ⋯ Trauma to the surrounding soft tissues affected the healing process. BMP-2 was expressed at all stages in both groups. We conclude that the healing process is disturbed by the fixing of a plate; that periosteum is one of the main sources of osteogenic cells; and that BMP-2 is an important regulator of morphogenesis.

Current interest in minimally invasive surgery has extended to the repair of craniomaxillofacial fractures, including subcondylar fractures of the mandible. Recent experience indicates that rigid fixation by the endoscopic approach is technically feasible, particularly for subcondylar fractures with lateral displacement of the proximal fragment. The technique is more difficult (although not impossible) for medially displaced fractures. ⋯ Because evidence suggests that the outcome of open reduction of subcondylar fractures may be better than the outcome when closed management is performed, techniques that minimize the morbidity of open reduction should allow better long-term results. It is hoped that the endoscopic approach for the repair of subcondylar fractures of the mandible will provide these benefits. The technique is summarized with step-by-step explanations.
