Multicenter StudyA clinical decision aid to discern patients without and with midfacial and mandibular fractures that require treatment (the REDUCTION-II study): a prospective multicentre cohort study.
To assess the diagnostic accuracy of physical examination findings and to construct clinical decision aids to discern emergency department patients without and with midfacial and mandibular fractures that require treatment. ⋯ The clinical decision aids successfully identified midfacial and mandibular trauma patients requiring active fracture treatment and so may be useful in preventing unnecessary radiological procedures in the future.

The treatment challenges of condylar fractures necessitated the production of several plate designs. Among the relatively new plate designs is the lambda plate, for which biomechanical and clinical data are lacking. The purpose of this study is to examine the rigidity of fixation achieved when the lambda plate is applied to different fractures of the condylar neck and base. ⋯ According to our study, the lambda plate offers better rigidity when applied as cranially as possible for condylar neck fractures. The lambda plate did not provide adequate fixation for base fractures. A second plate at the sigmoid notch should be considered to achieve better stabilization along the fracture if the lambda plate is eventually used.

Comparison of different treatment techniques in the mandibular condyle fracture.
Mandibular condyle fractures can be treated conservatively by intermaxillary fixation (IMF) or by open reposition and internal fixation. Although many studies have discussed for the optimal treatment method, the issue remains controversial. In this study, we aimed to compare conservative techniques in the mandibular condyle fractures. ⋯ The use of IMF with an occlusal splint is a more conservative and acceptable treatment modality than open reduction in selected cases.

Standard treatment of mandibular nonunion includes debridement and application of maxillomandibular or rigid internal fixation techniques, with adjunctive bone grafting when necessary. Frequently described in the orthopaedic literature, low-intensity pulsed ultrasound therapy (LIPUS) is a noninvasive treatment modality used to accelerate healing of fresh fractures and established nonunions. The purpose of this study was to conduct a systematic review to determine the extent of LIPUS study in the treatment of mandibular nonunions to identify whether LIPUS represents an effective nonsurgical alternative or adjunct for nonunion management. ⋯ This report reviews published literature on mandibular nonunions, and the evidence of LIPUS use in long bone nonunions. There are no known studies presenting LIPUS treatment of mandible fracture nonunions. However, on the basis of published orthopaedic data, LIPUS therapy could be considered as an adjunct or alternative to traditional surgical management of select mandible fracture nonunions.
