Sildenafil is the first phosphodiesterase type 5 inhibitor (PDE-5), used in the management of erectile dysfunction (ED). The effectiveness and safety of the original drug have been studied most extensively among ED medications. Sildenafil has been successfully used for ED of different etiology and severity. To date, this PDE-5 remains the drug of choice for many patients with erectile dysfunction.

Health-Related Lifestyle Factors and Sexual Dysfunction: A Meta-Analysis of Population-Based Research.
Sexual dysfunction is a common problem among men and women and is associated with negative individual functioning, relationship difficulties, and lower quality of life. ⋯ Results provide compelling evidence that cigarette smoking, alcohol, and physical activity are important for sexual dysfunction. Insufficient research was available to draw conclusions regarding risk factors for premature ejaculation or for cannabis use as a risk factor. Association Between Opioid Use and Risk of Erectile Dysfunction: A Systematic Review and Meta-Analysis.
Opioid analgesics have been widely used to relieve chronic pain conditions; however, a connection between opioid analgesic administration and increased susceptibility to erectile dysfunction (ED) has been hypothesized. ⋯ Evidence from the included observational studies indicated that men with opioid use had a significantly increased risk of ED. ReviewEfficacy and Safety of Sildenafil in Men With Sexual Dysfunction and Spinal Cord Injury.
Spinal cord injury (SCI) is estimated to affect approximately 276,000 individuals in the United States. Since 2010, the mean age of individuals at the time of the SCI has been 42 years, with nearly 80% of cases involving men. This means that individuals with SCI generally are young men who typically place a great deal of importance on normal sexual and reproductive function. ⋯ Sildenafil significantly improves erections, intercourse success, and ejaculation frequency vs placebo, including in men with complete SCI. Sildenafil is an effective and well-tolerated treatment for sexual dysfunction in men with SCI. Lower urinary tract symptoms (LUTSs) and erectile dysfunction (ED) are substantial health concerns with a significant impact on the overall male quality of life. ⋯ In this manuscript, we report on all the available evidence linking erectile dysfunction and lower urinary tract symptoms. Our findings suggest the existence of a strong relationship between these two conditions. On the basis of these findings, we recommend that clinicians always explore both conditions in male patients presenting with either of symptoms.
