Interventions for Tobacco Smoking Cessation in Adults, Including Pregnant Persons: US Preventive Services Task Force Recommendation Statement.
Tobacco use is the leading preventable cause of disease, disability, and death in the US. In 2014, it was estimated that 480 000 deaths annually are attributed to cigarette smoking, including second hand smoke exposure. Smoking during pregnancy can increase the risk of numerous adverse pregnancy outcomes (eg, miscarriage and congenital anomalies) and complications in the offspring (including sudden infant death syndrome and impaired lung function in childhood). In 2019, an estimated 50.6 million US adults (20.8% of the adult population) used tobacco; 14.0% of the US adult population currently smoked cigarettes and 4.5% of the adult population used electronic cigarettes (e-cigarettes). Among pregnant US women who gave birth in 2016, 7.2% reported smoking cigarettes while pregnant. ⋯ The USPSTF recommends that clinicians ask all adults about tobacco use, advise them to stop using tobacco, and provide behavioral interventions and FDA-approved pharmacotherapy for cessation to nonpregnant adults who use tobacco. (A recommendation) The USPSTF recommends that clinicians ask all pregnant persons about tobacco use, advise them to stop using tobacco, and provide behavioral interventions for cessation to pregnant persons who use tobacco. No evidence of differences in smoking levels, nicotine dependence, carbon monoxide or motivational indices between cigarette smokers and cigarette + e-cigarette dual users in two samples.
Concurrent use of electronic cigarettes (e-cigarettes) as an alternative source of nicotine may lead to lower smoke exposure, tobacco addiction and reinforcing value of conventional combustible cigarettes. In two different cohorts, we characterized smokers, dual cigarette/e-cigarette users, and e-cigarette-only users in terms of sociodemographic, motives and patterns of e-cigarette use. Differences in smoking-related characteristics and reinforcing value of cigarettes between smokers and e-cigarette dual users were also examined. ⋯ Compared to e-cigarette-only individuals, dual users from the two cohorts reported a greater use of e-cigarettes for quitting smoking, but dual product consumers exhibited no significant differences in motivation for change relative to combustible-only smokers. Relative to smokers, e-cigarette dual users did not show lower levels of smoking, lower severity of cigarette addiction and reinforcing value of cigarettes in these samples. These findings suggest that concurrent e-cigarette use does not affect combustible tobacco use or motivation, or reduce its harms.

Background: The (COVID-19) pandemic resulted in sudden disruption of routine clinical care necessitating rapid transformation to maintain clinical care while safely reducing virus contagion. Introduction: Memorial Sloan Kettering (MSK) experienced a rapid evolution from delivery of in-person cessation counseling services to virtual telehealth treatments for our tobacco-dependent cancer patients. Aim: To examine the effect of rapid scaling of tobacco treatment telehealth on patient engagement, as measured by attendance rates for in-person counseling visits versus remote telehealth counseling visits. ⋯ Discussion: Clinical, Information Technology (IT), and hospital system barriers were successfully addressed for most cancer patients seeking individual telehealth treatment. Group telehealth services were found to be feasible and acceptable. Conclusions: MSK's rapid leap into virtual care delivery mitigated disruption of tobacco treatment services and demonstrated strong feasibility and acceptance for managing complex tobacco-dependent patients.

This study assessed the association of exclusive and concurrent use of cigarettes, electronic nicotine delivery systems (ENDS), and cigars with ever and past 12-month wheezing symptoms among a nationally representative sample of US adult current tobacco users. ⋯ Cross-sectional associations with ever and past 12-month wheezing symptoms were found to be the strongest among cigarette users, exclusively or in combination. Future longitudinal research is needed to better understand how cigarette use interacts with other tobacco and nicotine products and contributes to respiratory symptoms.
