ReviewRole of novel drugs in sedation outside the operating room: dexmedetomidine, ketamine and remifentanil.
Progress in medical technology, diagnostic procedures and imaging techniques results in a growing demand for well tolerated sedation regimens, devoid of respiratory and haemodynamic complications. Moreover, rapid turnover time dictates the need for rapid onset of effect and rapid recovery. Recent literature regarding the use of dexmedetomidine, ketamine and remifentanil for sedation outside the operating room is reviewed. As procedural sedation is often performed by nonanaesthesiologists, articles from journals other than anaesthesiology journals are also included. ⋯ Recent literature is reviewed regarding dexmedetomidine, ketamine and remifentanil for its use outside the operating room. Sedationists have to keep in mind the pharmacokinetics and pharmacodynamics of the currently used agents in adults and children.

ReviewA review of the use of adjunctive therapies in severe acute asthma exacerbation in critically ill children.
Asthma is a common and potentially life threatening childhood condition. Asthma involves not only chronic airway remodeling, but may also include frequent exacerbations resulting from bronchospasm, edema, and mucus production. ⋯ Rarely, these measures are not successful and following the initiation of invasive mechanical ventilation, inhaled anesthetics or extracorporeal life support may be required. In this review, we discuss the mechanisms and evidence for adjunctive therapies in the setting of severe acute asthma exacerbations in children.

ReviewA review of the surface and internal anatomy of the caudal canal in children.
The anatomy of the sacral hiatus and caudal canal is prone to significant variation, yet studies assessing this in the pediatric population remain limited. Awareness of the possible anatomical variations is critical to the safety and success of caudal epidural blocks, particularly when image guidance is not employed. ⋯ A literature search using three electronic databases and standard pediatric and anatomy reference texts was conducted yielding 24 primary and seven secondary English-language sources. Appreciating that our current landmark-guided approaches to the caudal canal are not well studied in the pediatric population is important for both clinicians and researchers.

ReviewGuanfacine extended release for the treatment of attention-deficit/hyperactivity disorder in children and adolescents.
Guanfacine extended release (GXR) is a selective α(2A)-adrenoreceptor agonist originally developed as an antihypertensive agent and now FDA approved for the treatment of attention-deficit/hyperactivity disorder (ADHD) as monotherapy and as adjunctive to psychostimulants in children and adolescents 6-17 years old. ⋯ GXR is an evidence-based treatment for ADHD in children and adolescents. Because this compound has a smaller effect size than psychostimulants for the symptoms of ADHD, it is generally considered a second-line treatment after the psychostimulants or in combination with psychostimulants. Evidence for efficacy is more robust in children than for adolescents. Because of its pharmacodynamic actions in prefrontal cortex, GXR shows considerable promise for other behavioral conditions frequently comorbid with ADHD and potential promise for emotional and behavioral dysregulation secondary to traumatic stress.

Review Meta AnalysisHypotonic versus isotonic fluids in hospitalized children: a systematic review and meta-analysis.
To determine whether the use of hypotonic vs isotonic maintenance fluids confers an increased risk of hyponatremia in hospitalized children. ⋯ In hospitalized children in intensive care and postoperative settings, the administration of hypotonic maintenance fluids increases the risk of hyponatremia when compared with administration of isotonic fluids. For patients on general wards, insufficient data are available based on the reviewed studies, and individual risk factors must be assessed.
