Developing a Transitions of Care Elective for Medical Students during the COVID-19 Pandemic and Beyond.
Health care in the United States is costly, fragmented, and often ineffective. Transitions of care (TOC), particularly from the inpatient to the outpatient setting, is an especially complicated time and one that is potentially fraught with errors that contribute to negative outcomes. The coronavirus 2019 pandemic exacerbated many of these challenges. In particular, vulnerable patient populations have experienced more barriers to successful care transitions. Effective care transitions should include interprofessional teamwork, robust patient education, and seamless communication among the various healthcare team members. Increasingly, medical schools are working toward graduating systems-ready physicians who demonstrate competency in the health system sciences and are able to operate effectively within the healthcare system, including being able to navigate complex transitions of care issues. Undergraduate medical education, however, continues to provide experiential learning in the traditional silos of inpatient versus outpatient medicine, so that learners do not have the opportunity to directly participate in transitions of care. Although transitions of care is a pivotal part of patient care, it is rarely taught at the undergraduate level, and when it is, it is typically relegated to the classroom setting. ⋯ Participation of students in TOC is a valuable educational experience and contributes a value-added component to patient care and interprofessional teamwork. Moreover, an appreciation of the failures of the current system is pivotal as learners start to reimagine, explore, and design improved patient-centered systems in the future.

The Padova Hospital Vascular Surgery Division is located in Veneto, one of the area of the Northern Italy most hit by the Coronavirus disease 2019 outbreak. The aim of this paper is to describe the protocols adopted and to evaluate their impact during the acute phase of Coronavirus spread, focusing on the management of elective and urgent/emergent surgery, outpatients activity, and also health staff preservation from intra-hospital Coronavirus disease 2019 infection. ⋯ Elective vascular surgery needs to be guaranteed as possible during Coronavirus disease 2019 outbreak. The number of truly emergent cases did not reduce, on the other side, Emergency Room accesses for non-emergent cases decreased. Our preliminary results seem to describe a scenario where, if the curve of the outbreak in the regional population is flattened, in association with appropriate hospitals containment rules, it may be possible to continue the activity of the Vascular Surgery Units and guarantee the minimal standard of care.

Here we compare Google searches of perinatal mental health-related terms during the U. ⋯ These insights offer important population-level considerations ahead of further U. K. restrictions, and should be imminently confirmed with epidemiological work.

Coronaviruses are RNA viruses classified into Alphacoronavirus, Betacoronavirus, Gammacoronavirus and Deltacoronavirus. Their name comes from their conformation with the observation of spicules forming a sort of crown. Coronaviruses seem to come from bats, and more particularly Alphacoronaviruses and Betacoronaviruses (the genus where zoonoses are observed), while birds are the source of Gammacoronaviruses and Deltacoronaviruses. ⋯ Most Alphacoronaviruses are species specific. They can be responsible for serious diseases such as the cat coronavirus, responsible for feline infectious peritonitis, transmissible gastroenteritis (GET) in piglets and porcine epidemic diarrhea (PED), porcine respiratory coronavirus (PRCV). Finally, a deltacoronavirus can be found in both pigs (PD CoV UKU15) and birds.

Hip fractures in elderly patients are a major cause of morbidity and mortality. Variability in length of hospital stay (LOS) was evident in this population. The coronavirus disease-2019 (COVID-19) pandemic led to prompt discharge of effected patients in order to reduce contagion risk. LOS and discharge destination in COVID-19 negative patients has not been studied. ⋯ LOS and time to rehabilitation were significantly shorter in the COVID group. Delirium was more common in this group, possibly due to negative effects of social distancing.
