Probiotic ingestion is associated with an increase in intestinal flora of useful bacteria, which contributes to the known protective effects it has on the intestinal barrier and thereby reducing infection. The present study aims to investigate the protective effect of Lactobacillus rhamnosus gg (LGG) as an important probiotic with gastrointestinal barrier strengthening effect in sepsis. ⋯ Prophylactic use of probiotics, such as LGG to reduce bacterial translocation and strengthen the immune system, is an inexpensive and effective method of treatment, and we recommend the repetition of studies supported by prospective clinical trials.

Fluid Response Evaluation in Sepsis Hypotension and Shock: A Randomized Clinical Trial.
Fluid and vasopressor management in septic shock remains controversial. In this randomized controlled trial, we evaluated the efficacy of dynamic measures (stroke volume change during passive leg raise) to guide resuscitation and improve patient outcome. ⋯ NCT02837731.

(1,3)-β-D-Glucan-based empirical antifungal interruption in suspected invasive candidiasis: a randomized trial.
(1,3)-β-D-Glucan has been widely used in clinical practice for the diagnosis of invasive Candida infections. However, such serum biomarker showed potential to guide antimicrobial therapy in order to reduce the duration of empirical antifungal treatment in critically ill septic patients with suspected invasive candidiasis. ⋯ In critically ill septic patients admitted to the ICU at risk of invasive candidiasis, a (1,3)-β-D-glucan-guided strategy could reduce the duration of empirical antifungal therapy. However, the safety of this algorithm needs to be confirmed in future, multicentre clinical trial with a larger population.

Efficacy and safety of landiolol, an ultra-short-acting β1-selective antagonist, for treatment of sepsis-related tachyarrhythmia (J-Land 3S): a multicentre, open-label, randomised controlled trial.
Tachycardia and atrial fibrillation frequently occur in patients being treated for sepsis or septic shock and have a poor prognosis. Treatments for tachyarrhythmias are often ineffective or contraindicated in this setting. We aimed to investigate the efficacy and safety of landiolol, an ultra-short-acting β-blocker, for treating sepsis-related tachyarrhythmias. ⋯ Ono Pharmaceutical Co.

Effect of dexmedetomidine on inflammation in patients with sepsis requiring mechanical ventilation: a sub-analysis of a multicenter randomized clinical trial.
Administration of dexmedetomidine has been reported to improve inflammatory response in animals. We explored the effects of administering dexmedetomidine on the levels of C-reactive protein (CRP) and procalcitonin, and thus on inflammation, in patients with sepsis enrolled in a randomized clinical trial. ⋯ Sedation using dexmedetomidine reduced inflammation in patients with sepsis requiring mechanical ventilation.
