Diagnosis of sepsis in the postoperative period is a challenge. Measurements of inflammatory markers, such as C-reactive protein (CRP), have been proposed in medical patients, but the interpretation of these values in surgical patients is more difficult. We evaluated the changes in blood CRP levels and white blood cell count in postoperative patients with and without infection. ⋯ CRP levels increase in the first week after major surgery but to a much larger extent in infected than in noninfected patients. Persistently high CRP levels after POD 4, especially when >100 mg/L, suggest the presence of a postoperative infection.

Analgesic efficacy of pre-operative stellate ganglion block on postoperative pain relief: a randomised controlled trial.
This small trial found that pre-operative stellate ganglion block before upper-limb orthopaedic surgery significantly reduced both 24 h tramadol consumption and pain at rest for up to 6 hours, although the later was of borderline clinical significance.

Randomized Controlled TrialEffects of peritoneal ropivacaine nebulization for pain control after laparoscopic gynecologic surgery.
To evaluate the effects of peritoneal cold nebulization of ropivacaine on pain control after gynecologic laparoscopy. ⋯ Cold nebulization of ropivacaine before surgery reduced postoperative pain and morphine consumption and was associated with earlier walking without assistance.
