Survival of hematological patients after discharge from the intensive care unit: a prospective observational study.
Although the survival rates of hematological patients admitted to the ICU are improving, little is known about the long-term outcome. Our objective was to identify factors related to long-term outcome in hematological patients after ICU discharge. ⋯ Both ICU care and post-ICU management determine the long-term outcome of hematological patients who are discharged alive from the ICU.

The development of acute kidney injury (AKI) is associated with poor outcome. The modified RIFLE (risk, injury, failure, loss of kidney function, and end-stage renal failure) classification for AKI, which classifies patients with renal replacement therapy needs according to RIFLE failure class, improves the predictive value of AKI in patients undergoing cardiac surgery. Our aim was to assess risk factors for post-operative AKI and the impact of renal function on short- and long-term survival among all AKI subgroups using the modified RIFLE classification. ⋯ AKI development after cardiac surgery is associated mainly with post-operative variables, which ultimately could lead to a worst RIFLE class. Staging at the RIFLE injury and RIFLE failure class is associated with higher short- and long-term mortality in our population.

ReviewAnesthesia for the young child undergoing ambulatory procedures: current concerns regarding harm to the developing brain.
Retrospective studies show that a single anesthesia exposure before age 3 may undermine language acquisition and abstract reasoning, and exposure to two or more anesthetics before age 2 almost doubles the risk of attention-deficit hyperactivity disorder, although in both cases causality has not yet been established.

Randomized Controlled Trial Comparative StudyReversal of Decreases in Cerebral Saturation in High-risk Cardiac Surgery.
To measure the incidence of cerebral desaturation during high-risk cardiac surgery and to evaluate strategies to reverse cerebral desaturation. ⋯ Cerebral desaturation in high-risk cardiac surgery is frequent but can be reversed most of the time resulting in a smaller desaturation load. A large randomized study will be needed to measure the impact of reversing cerebral desaturation on patient's outcome.

Comparative StudyMicrovascular training of medical students and surgeons - a comparative prospective study.
Since microsurgical experience remains the most important skill in various surgical fields it should be in general part of the training program, either in medical studies or residency. This study compares the results of microsurgical education after completion of a comprehensive microsurgical course program between students and surgeons. 59 clinical medical students and 19 surgeons participated at a weekly 14-day microsurgical training course. Two examiners assessed the participants independently and blinded. ⋯ Prior to participation the students described an ambition to a career in a specialty performing microsurgery in 23 cases, after the microsurgical course following completion 50 participants worked in a surgical field. Microsurgical education can be part of undergraduate medical studies. The microsurgery training course influenced the students' ambition in relation to microsurgery as a future career.
