Multicenter StudyFrozen elephant trunk does not increase incidence of paraplegia in patients with acute type A aortic dissection.
We seek to assess the safety of total arch replacement with frozen elephant trunk for acute type A aortic dissection in respect to the risks of operative mortality, stroke, and paraplegia using an international multicenter registry (ARCH). ⋯ The use of frozen elephant trunk for acute type A aortic dissection does not appear to increase the risk of paraplegia in appropriately selected patients at experienced centers. The exact risk factors for paraplegia remain to be determined.

Risk Analysis and Outcomes of Postoperative Renal Failure After Aortic Valve Surgery in the United States.
Postoperative renal failure (RF) compromises early outcomes in cardiac surgery. In contrast, long-term survival and progression of RF after aortic valve replacement (AVR) with or without coronary artery bypass graft surgery (CABG) remain undefined. ⋯ Postoperative RF after AVR with or without CABG was associated with higher early and late mortality, which further increased with a new requirement for dialysis. Once postoperative RF develops, preoperative renal dysfunction does not increase early mortality; however, it predicts late survival. Preoperative renal function is associated with progression of postoperative RF to dialysis.

Multicenter StudyUnderweight is associated with inferior short and long-term outcomes after MitraClip implantation: Results from the German TRAnscatheter mitral valve interventions (TRAMI) registry.
Underweight and obesity represent classical risk factors for adverse outcome in patients treated for cardiovascular disease. ⋯ Compared to other weight groups, underweight patients undergoing MitraClip implantation are exposed to increased rates of procedural failure, bleeding and low cardiac output as well as increased short- and long-term mortality rates and should therefore be carefully discussed in the heart-team.
