ReviewMachine Vision and Image Analysis in Anesthesia: Narrative Review and Future Prospects.
Machine vision describes the use of artificial intelligence to interpret, analyze, and derive predictions from image or video data. Machine vision-based techniques are already in clinical use in radiology, ophthalmology, and dermatology, where some applications currently equal or exceed the performance of specialty physicians in areas of image interpretation. While machine vision in anesthesia has many potential applications, its development remains in its infancy in our specialty. ⋯ The performance and potential uses of machine vision for anesthesia will only grow with the advancement of underlying machine vision algorithm technical performance developed outside of medicine, such as convolutional neural networks and transfer learning. This article summarizes recently published works of interest, provides a brief overview of techniques used to create machine vision applications, explains frequently used terms, and discusses challenges the specialty will encounter as we embrace the advantages that this technology may bring to future clinical practice and patient care. As machine vision emerges onto the clinical stage, it is critically important that anesthesiologists are prepared to confidently assess which of these devices are safe, appropriate, and bring added value to patient care.

Observational StudyPrediction of unfavorable outcomes in community-acquired bacteremia by SIRS, SOFA and qSOFA scores.
Sepsis diagnostic and prognostic scoring systems have changed over time. It remains uncertain which scoring system is the best predictor of unfavorable outcomes. We aimed to evaluate prediction of community-acquired bacteremia (CAB) outcomes using on-admission systemic inflammatory response syndrome (SIRS), sequential organ failure assessment (SOFA) and quick sequential organ failure assessment (qSOFA). ⋯ qSOFA≥2 was associated with highest probability of unfavorable outcome, but dichotomized SOFA was more precise at high vs. low-risk distinction. Consecutive use of dichotomized qSOFA and SOFA on admission of adults with CAB enables fast and reliable identification of patients at high (qSOFA≥2, risk ~≥35%), moderate (qSOFA 0-1, SOFA≥2, risk ~10%), and low risk (qSOFA 0-1, SOFA 0-1, risk 1-2%) of subsequent unfavorable events.

Clinical TrialMinimizing Lung Injury During Laparoscopy in Head-Down Tilt: A Physiological Cohort Study.
Increased intra-abdominal pressure during laparoscopy induces atelectasis. Positive end-expiratory pressure (PEEP) can alleviate atelectasis but may cause hyperinflation. Cyclic opening of collapsed alveoli and hyperinflation can lead to ventilator-induced lung injury and postoperative pulmonary complications. We aimed to study the effect of PEEP on atelectasis, lung stress, and hyperinflation during laparoscopy in the head-down (Trendelenburg) position. ⋯ Higher PEEP levels during laparoscopy in the head-down position facilitate lung-protective ventilation. Atelectasis and lung stress are reduced in the absence of global alveolar hyperinflation.

Pain after cardiac surgery via median sternotomy can be difficult to treat, and if inadequately managed can lead to respiratory complications, prolonged hospital stays and chronic pain. ⋯ The analgesic regimen for cardiac surgery via sternotomy should include paracetamol and NSAIDs, unless contraindicated, administered intra-operatively and continued postoperatively. Intra-operative magnesium and dexmedetomidine infusions may be considered as adjuncts particularly when basic analgesics are not administered. It is not clear if combining dexmedetomidine and magnesium would provide superior pain relief compared with either drug alone. Parasternal block/surgical site infiltration is also recommended. However, no basic analgesics were used in the studies assessing these interventions. Opioids should be reserved for rescue analgesia. Other interventions, including cyclo-oxygenase-2 specific inhibitors, are not recommended because there was insufficient, inconsistent or no evidence to support their use and/or due to safety concerns.

The Effect of Virtual Glasses Application on Pain and Anxiety During Episiotomy Repair: Randomized Controlled Trial.
In the literature, the efficacy of virtual glasses on acute pain and anxiety has been investigated, and no study has been found on its effect on pain and anxiety during episiotomy repair. ⋯ Virtual glasses are more effective than skin to skin contact and control methods in decreasing pain and reducing anxiety during episiotomy repair. Additionally, virtual glasses may reduce the need for pharmacological medication due to this reduction in pain and anxiety during episiotomy repair.
