[Artificial Intelligence: Infrastructures and Prerequisites at European Level].
The application of artificial intelligence (AI) is often associated with the use of large amounts of data for the construction of AI models and algorithms. This data should ideally comply with the FAIR Data principles, i.e. being findable, accessible, interoperable and reusable. However, the handling of health data poses a particular challenge in this context. ⋯ We discuss the current situation but also the obstacles for a wider application of AI in medicine in Europe and give suggestions how to solve the different issues. The article covers different subjects like data protection, research data infrastructures and approval of medical products. Finally, this article shows how it can nevertheless be possible to establish a secure and at the same time effective handling of data for use in AI at the European level despite its unneglectable difficulties.

An infraclavicular catheter is a very efficient technique to provide excellent perioperative anesthesia/analgesia for upper limb surgery. However, complications can occur and are dependent on the technique used. We report the inadvertent placement of an infraclavicular catheter in the interscalene region when an ultrasound-guided infraclavicular catheter was threaded cranially. We proposed some strategies to avoid the occurrence of complications when performing this block.

Large studies comparing totally minimally invasive oesophagectomy (TMIE) with laparoscopically assisted (hybrid) oesophagectomy are lacking. Although randomized trials have compared TMIE invasive with open oesophagectomy, daily clinical practice does not always resemble the results reported in such trials. The aim of the present study was to compare complications after totally minimally invasive, hybrid and open Ivor Lewis oesophagectomy in patients with oesophageal cancer. ⋯ Compared with hybrid and open Ivor Lewis oesophagectomy, TMIE resulted in a lower pneumonia rate, a shorter duration of hospital stay, but higher anastomotic leakage rates. Therefore, no clear advantage was seen for either TMIE, hybrid or open Ivor Lewis oesophagectomy when performed in daily clinical practice.

High-frequency (10-kHz) spinal cord stimulation (SCS) continues to be an emerging therapy in chronic pain management. The same complications that plagued earlier SCS systems may affect newer stimulation technologies, although there is limited data on the type of complications and surgical management of these complications. ⋯ This retrospective 5-year analysis of complications from10-kHz SCS provides a real-world assessment of safety data unique for this stimulation modality. This analysis may help inform future clinical decisions, lead to device enhancement and optimization, and improve mitigation of risks to provide safe and efficacious use of 10-kHz SCS.
