Scorpion antivenom (SAV) is specific antidote to scorpion venom..SAV did not prevent the cardiovascular morbidity and mortality (autonomic storm), hence its utility in the management for severe scorpion envenomingmay be limited. Since 1983 the advent of prazosin revolutionized the management of severe scorpion sting. Since 2002 SAV against Indian red scorpion (IRS) for the treatment of scorpion sting cases is available at primary health centers. We compared the effects of SAV Vs Prazosin (PRA) in the management of severe scorpion stung cases at rural setting in a non-randomised open label manner. ⋯ We found that SAV is no more effective to alleviate or reverse the cardiovascular effects of scorpion venom actions in severe case as against prazosin prevents and cures the cardiovascular manifestations in a severe scorpion envenomation. Therefore role of SAV in severe scorpoin venomation needs to be relooked and prazosin needs to be a standard of care in such cases to overcome the autonomic storm.

Case ReportsAdrenaline-induced digital ischaemia reversed with phentolamine.
Adrenaline autoinjectors are widely prescribed for self-treatment of anaphylactic syndromes. Accidental misfire into digits can cause prolonged significant ischaemia with serious outcomes. Phentolamine is a readily available treatment, which is easily and safely given in the emergency setting.

Tolterodine and tamsulosin for treatment of men with lower urinary tract symptoms and overactive bladder: a randomized controlled trial.
Men with overactive bladder and other lower urinary tract symptoms may not respond to monotherapy with antimuscarinic agents or alpha-receptor antagonists. ⋯ These results suggest that treatment with tolterodine ER plus tamsulosin for 12 weeks provides benefit for men with moderate to severe lower urinary tract symptoms including overactive bladder. Phaeochromocytoma is a rare catecholamine-secreting neuroendocrine tumour with a high cardiovascular morbidity and mortality if left untreated. Surgical resection is the only curative therapy. During surgery there is a high risk of massive release of catecholamines, which can result in potentially fatal hypertensive crises and cardiac arrhythmias. ⋯ Guidelines for the preoperative medical management of the patient with a phaeochromocytoma are mainly based on retrospective studies and case reports. We reviewed the relevant literature on this subject. In addition, we compared the several preoperative treatment protocols of the eight university medical centres in the Netherlands.

To assess the clinical role of corticosteroids in the medical expulsive therapy of symptomatic distal ureteral stones. ⋯ When the medical expulsive therapy for symptomatic distal ureteral stones is considered, the use of steroids (deflazacort) proves efficient only when administered together with alpha(1)-blockers (tamsulosin). In addition, tamsulosin used on its own as a medical expulsive therapy can be considered as an alternative treatment for those patients who are not suitable for steroid therapy, as it is generally efficient.
