Reliability and validity of the Manchester Triage System in a general emergency department patient population in the Netherlands: results of a simulation study.
To assess the reliability and validity of the Manchester Triage System (MTS) in a general emergency department patient population. ⋯ Inter-rater reliability is "moderate" to "substantial" and test-retest reliability is high. The reliability of the MTS is not influenced by nurses' work experience. Undertriage mainly occurs in the MTS categories orange and yellow. The MTS is more sensitive for children who need immediate or urgent care than for other patients in the emergency department.

EMDOC (Emergency Department overcrowding) Internet-based safety net research.
Emergency Department (ED) overcrowding is a national crisis with few prospective data to document its occurrence. The objective of this study was to prospectively collect data on variables involved in Emergency Department overcrowding (EMDOC) using an Internet-based data entry model. A prospective observational Internet-based study involving 18 hospitals over a 13-month period was designed. ⋯ The mean number of makeshift beds was 3.1. There was no single variable that was noted to define or predict overcrowding. Documentation of factors involved in ED overcrowding found that overcrowding was not just an ED problem, but a problem that occurs due to overcrowding in the entire institution.

Hospital variability of out-of-hospital cardiac arrest survival.
Previous literature has identified patient and emergency medical services (EMS) system factors that are associated with survival of out-of-hospital cardiac arrest patients. ⋯ Survival to discharge of resuscitated adult out-of-hospital cardiac arrest patients may vary by receiving hospital. A hospital's ratio of beds to nurse and several patient/case f actors are correlated with survival. Further research is warranted to investigate how this may affect resuscitation care, EMS transport policy, and research design.

To determine medical and nursing staffing levels in emergency departments in England and Wales. ⋯ There is great variability in staffing numbers in similar-sized departments, and most departments are understaffed in comparison with the recommendations of the British Association for Emergency Medicine to achieve the 4 h target at the 90% level.

Impact of an ultrabroadband emergency department telemedicine system on the care of acutely ill patients and clinicians' work.
To evaluate whether introduction of an emergency department (ED) telemedicine system changed patient management and outcome indicators and to investigate clinicians' perceptions of the impact of the system on care provided and on their work. ⋯ The ViCCU appears most effective for moderate trauma patients, with associated reductions in admissions and transfers. Large-scale trials of telemedicine systems that include measurements of both patient care and impact on clinicians' work are required.
