Nebulized lidocaine decreases the discomfort of nasogastric tube insertion: a randomized, double-blind trial.
Nasogastric tube insertion is a common emergency department (ED) procedure that is associated with considerable patient discomfort. The safety and efficacy of nebulized lidocaine for upper airway anesthesia have previously been demonstrated. We determine whether nebulized lidocaine administered before nasogastric tube insertion significantly reduces patient discomfort. ⋯ Nebulized lidocaine decreases the discomfort of nasogastric tube insertion and should be considered before passing a nasogastric tube. An increased frequency of epistaxis, however, may be associated with its use.

Randomized Controlled Trial Multicenter Study Clinical TrialEvaluation of a low-dose lidocaine iontophoresis system for topical anesthesia in adults and children: a randomized, controlled trial.
Commonly used classes of topical anesthetics require 30 to 60 minutes to provide effective anesthesia. A new low-dose lidocaine iontophoresis system (LDLIS) may provide topical anesthesia in 10 minutes at a lower dose than previous systems, thereby limiting adverse events. ⋯ In this study of adults and children, the LDLIS provided effective topical anesthesia for venipuncture and venous cannulation within 10 minutes.

The use of remifentanil in routine clinical practice during the induction and maintenance of general anesthesia as well as the quality of awakening after anesthesia and post-operative pain management have been assessed. ⋯ This Drug Utilisation Research study demonstrated that the use of remifentanil according to its peculiar pharmacological profile, such as potent opioid with rapid onset and offset of action, the synergistic effect with propofol and the right management of post-operative pain are widely consolidated in Italian clinical practice.

Ondansetron, dexamethasone, droperidol and total intravenous anesthesia (TIVA) all have a roughly similar, productive effect to reduce postoperative nausea and vomiting (PONV) by about one third.

Sevoflurane for interventional neuroradiology procedures is associated with more rapid early recovery than propofol.
Sevoflurane and propofol are both suitable for neuroanesthesia but have not previously been compared as maintenance agents for long duration (one to five hours) procedures. ⋯ The use of sevoflurane for maintenance of anesthesia for prolonged neuroradiological procedures is associated with more rapid early recovery than propofol and is associated with similar side effects. Sevoflurane and propofol can both be recommended for these procedures. The clinical benefit of the more rapid recovery with sevoflurane is unknown.
