AbstractHuman error has identified as a major source of ABO incompatibility-related transfusion fatalities. Frequency of ABO-incompatible transfusion has been reported to range from 2.5 to 253 per 100,000 transfusion units. Transfusion error has developed various occasions, for example, wrong specimen and patients, specimen exchange and mistake of ABO compatibility test. In laboratory examination, many errors were occurred during night shift, because it was necessary to established the examination system. We discussed about examination for prevent of blood transfusion errors.
