To determine the advantages and disadvantages of the use of diclofenac sodium instead of meperidine for postcesarean section pain control. ⋯ Diclofenac alone was safe and effective for the majority of the patients and it decreased the opioid requirements in the remaining patients.

Randomized Controlled TrialThe effects of the tocolytics atosiban and nifedipine on fetal movements, heart rate and blood flow.
The choice of first-line tocolytic agent is a topic of worldwide debate. The oxytocin receptor antagonist atosiban and the calcium antagonist nifedipine appear to be effective in postponing delivery. However, information is lacking on their possible effects on the fetal biophysical profile. ⋯ This study demonstrates for the first time the direct effects of atosiban on fetal movement, heart rate and blood flow. Tocolysis with either atosiban or nifedipine combined with betamethasone administration appears to have no direct fetal adverse effects.

Randomized Controlled TrialEfficacy of tranexamic acid in reducing blood loss after cesarean section.
To assess the efficacy and safety of tranexamic acid in reducing blood loss at caesarian section (CS). ⋯ Tranexamic acid statistically reduces blood loss from end to 2 h after CS and its use was not associated with any side effects or complications. Consequently, tranexamic acid can be used safely and effectively to reduce bleeding resulting from CS.

Randomized Controlled Trial Multicenter StudySide effects of oral misoprostol for the prevention of postpartum hemorrhage: results of a community-based randomised controlled trial in rural India.
To investigate the side effects of 600 microg oral misoprostol given for the mother and the newborn to prevent postpartum hemorrhage (PPH). ⋯ Misoprostol is associated with a significant increase in postpartum maternal shivering and fever with no side effects for the newborn. Given its proven efficacy for the prevention of PPH, the benefits of misoprostol are greater than the associated risks.

Randomized Controlled TrialEffect of oxytocics on the blood pressure of normotensive Nigerian parturients.
The single most common direct obstetric disorder accounting for 25% of all maternal deaths globally is severe hemorrhage, generally occurring postpartum. Nearly all these deaths occur in the developing world. The role of oxytocic drugs in the management of the third stage of labor as a strategy to reduce maternal mortality has been emphasized. However, the adverse effects of these oxytocic agents, in particular ergometrine, have not been properly evaluated in our environment. ⋯ These results suggest that ergometrine may be safe in normotensive parturients but hazardous in hypertensive parturients in whom oxytocin would be a safer option.
