Reducing tachycardia in septic shock patients: do esmolol and ivabradine have a chronotropic effect only?
An elevated heart rate (HR) often persists in resuscitated septic shock patients, increasing the risk of mortality. Several drugs for HR control, such as esmolol and ivabradine, have been tested in the recent years, but their benefit on the overall cardiovascular system is still under investigation. The aim of this study is to investigate the hemodynamic effects of the two drugs in a protocol of polymicrobial septic shock and resuscitation, mainly focusing on the vascular function. ⋯ Septic shock induced a severe cardiac and vascular disarray, only partially resolved by resuscitation. The administration of esmolol, but not ivabradine, was beneficial both for cardiac and vascular function, thereby its adjunction to standard therapies could help to improve patient's condition and optimize the resuscitation strategies. Clinical Relevance-This study shows a potential beneficial effect of esmolol on the arterial tree.

Investigating into segmentation methods for diagnosis of respiratory diseases using adventitious respiratory sounds.
Respiratory condition has received a great amount of attention nowadays since respiratory diseases recently become the globally leading causes of death. Traditionally, stethoscope is applied in early diagnosis but it requires clinician with extensive training experience to provide accurate diagnosis. Accordingly, a subjective and fast diagnosing solution of respiratory diseases is highly demanded. ⋯ Specifically, 14 combinations of respiratory sound segments were compared and we found segmentation plays an important role in classifying different respiratory conditions. The classifier with best performance (accuracy = 0.88, precision = 0.91, recall = 0.87, specificity = 0.91, F1-score = 0.81) was trained with features extracted from the combination of early inspiratory phase and entire inspiratory phase. To our best knowledge, we are the first to address the challenging multi-classification problem.

Phenotyping Cognitive Impairment using Graphomotor and Latency Features in Digital Clock Drawing Test.
The Clock Drawing Test, where the participant is asked to draw a clock from memory and copy a model clock, is widely used for screening of cognitive impairment. The digital version of the clock test, the digital clock drawing test (dCDT), employs accelerometer and pressure sensors of a digital pen to capture time and pressure information from a participant's performance in a granular digital format. ⋯ Our results show that the clusters are not significantly different in terms of age and gender, but did significantly differ in terms of education, Mini-Mental State Exam scores, and cognitive impairment diagnoses. This study shows that features extracted from digital clock drawings can provide important information regarding cognitive reserve and cognitive impairments.

A Novel Mainstream Capnometer System for Non-invasive Positive Pressure Ventilation.
Capnometry is a method to measure carbon dioxide (CO2) in exhaled gas and it has been used to monitor patient respiratory status. CO2 monitoring is also used for patients receiving non-invasive positive pressure ventilation (NPPV) therapy during mechanical ventilation. Ventilators actively dilute exhaled gas during non-invasive ventilation. ⋯ Therefore, we developed NPPV cap-ONE mask, which accurately measures CO2 in exhaled gas during non-invasive ventilation. In this study, we evaluated the basic performance of the NPPV cap-ONE mask system. The NPPV cap-ONE mask system could accurately measure CO2 in exhaled gas comparing to the conventional device in this study.

Robust Modelling of Reflectance Pulse Oximetry for SpO2 Estimation.
Continuous monitoring of blood oxygen saturation levels is vital for patients with pulmonary disorders. Traditionally, SpO2 monitoring has been carried out using transmittance pulse oximeters due to its dependability. However, SpO2 measurement from transmittance pulse oximeters is limited to peripheral regions. ⋯ The proposed solution produces an average mean absolute error of 1.81% on unseen patients which is well within the clinically permissible error of 2%. Two statistical tests were conducted to establish the effectiveness of the proposed method. Clinical relevance- The proposed method ameliorates our current understanding of reflectance based pulse oximetry and provides a method to estimate SpO2 from reflectance pulse oximeters.
