Hepatopulmonary syndrome (HPS) is an important cause in a patient with hypoxemia and CLD. HPS in patients with CLD modifies the line of management and determines the prognosis of the disease. HPS is characterized by arterial de-oxygenation, intrapulmonary vascular dilatation (IPVD) in the setting of liver disease. Bilateral Cadaveric Lobar Lung Transplantation (BCLLT) can expand the donor pool in cases where donors may be unsuitable for whole lung donation due to viability of the lower lobes and yet have upper lobes suitable for transplantation in patients of small stature or reduced pleural space.
