Effects of ranolazine on recurrent cardiovascular events in patients with non-ST-elevation acute coronary syndromes: the MERLIN-TIMI 36 randomized trial.
Ranolazine is a novel antianginal agent that reduces ischemia in patients with chronic angina but has not been studied in patients with acute coronary syndromes (ACS). ⋯ The addition of ranolazine to standard treatment for ACS was not effective in reducing major cardiovascular events. Ranolazine did not adversely affect the risk of all-cause death or symptomatic documented arrhythmia. Our findings provide support for the safety and efficacy of ranolazine as antianginal therapy.

Association of CFH Y402H and LOC387715 A69S with progression of age-related macular degeneration.
Studies have reported that single-nucleotide polymorphisms in the genes CFH and LOC387715 are associated with age-related macular degeneration (AMD). ⋯ Common polymorphisms in the genes CFH and LOC387715 are independently related to AMD progression after adjustment for other known AMD risk factors. Presence of these polymorphisms plus smoking [corrected] and body mass index of 25 or higher, controlling for AREDS vitamin-mineral treatment, identifies [corrected] patients who are highly susceptible to developing advanced states [corrected] of this visually disabling disease.

A predictive index for renal replacement therapy (RRT; hemodialysis or continuous venovenous hemodiafiltration) after cardiac surgery may improve clinical decision making and research design. ⋯ RRT after cardiac surgery is predicted by readily available preoperative information. A simple predictive index based on this information discriminated well between low- and high-risk patients in derivation and validation cohorts. The index had improved generalizability when used to predict likelihood ratios for RRT.

With routine childhood vaccination using heptavalent pneumococcal conjugate vaccine, one concern has been the potential for emergence and expansion of replacement disease caused by serotypes not contained in the heptavalent conjugate vaccine. ⋯ Alaska Native children are experiencing replacement invasive pneumococcal disease with serotypes not covered by heptavalent pneumococcal conjugate vaccine. The demonstration of replacement invasive pneumococcal disease emphasizes the importance of ongoing surveillance and development of expanded valency vaccines.
