To evaluate the effectiveness of Tai Chi Chuan in fall prevention in elderly people living at home with a high risk of falling. ⋯ These results suggest that Tai Chi Chuan may not be effective in elderly people at a high risk of falling who live at home.

To examine the effect of a multicomponent intervention on pain and function after orthopedic surgery. ⋯ The intervention improved postoperative pain, reduced chronic pain, and improved function.

To develop and characterize an automated syndromic surveillance mechanism for early identification of older emergency department (ED) patients with possible life-threatening infection. ⋯ A simple clinical informatics algorithm can detect infection in elderly patients in real time with high specificity. The utility of this tool for research and clinical care may be substantial.

To explore barriers to multiple advance care planning (ACP) steps and identify common barrier themes that impede older adults from engaging in the process as a whole. ⋯ Perceiving ACP to be irrelevant was the barrier theme most often endorsed at every ACP step. Other barriers were endorsed at specific steps. Understanding ACP barriers may help clinicians prioritize and address them and may also provide a framework for tailoring interventions to improve ACP engagement.

To examine the association between persistent delirium and 1-year mortality in newly admitted postacute care (PAC) facility patients with delirium who were followed regardless of residence. ⋯ In patients who were delirious at the time of PAC admission, persistent delirium was a significant independent predictor of 1-year mortality.
