Comparative StudyA comparison of 3 radiographic scales for the prediction of delayed ischemia and prognosis following subarachnoid hemorrhage.
Delayed cerebral ischemia is a major cause of morbidity and death following aneurysmal subarachnoid hemorrhage and requires timely intervention for a successful outcome to be achieved. In this study the investigators compared the commonly used Fisher scale with 2 newer radiographic scales for the prediction of vasospasm, delayed infarction, and poor outcome. ⋯ Although the modified Fisher and Claassen scales have yet to be prospectively validated, the authors' findings suggest that the clinical performance of these systems is superior to that of the Fisher scale.

Case Reports Clinical TrialEffect of intracranial pressure on the diameter of the optic nerve sheath.
The subarachnoid space around the optic nerve in the orbit can be visualized using T2-weighted MR imaging with the fat-saturation pulse sequence. The optic nerve sheath (ONS) diameter can be estimated by measuring the outer diameter of the subarachnoid space. Dilated ONS is associated with idiopathic intracranial hypertension and hydrocephalus, and is believed to reflect increased intracranial pressure (ICP). The relationship between dilated ONS and ICP is unclear because of the difficulty in obtaining noninvasive measurements of ICP. The authors investigated the relationship between subdural pressure measured at the time of surgery and ONS diameter measured on MR images in patients with chronic subdural fluid collection. ⋯ Increased ONS diameter measured on coronal orbital thin-slice fat-saturated T2-weighted MR images is a strong indicator of increased ICP, and helps to differentiate between passive subdural fluid collection due to brain atrophy and subdural hygroma with increased ICP.

Clinical TrialStereotactic drainage and Gamma Knife radiosurgery of cystic brain metastasis.
Treatment options for patients with brain metastasis include tumor resection, whole-brain radiation therapy, and radiosurgery. A single treatment is not useful in cases of multiple tumors, of which at least 1 is a cystic tumor. The purpose of this study was to assess the role of stereotactic drainage and Gamma Knife surgery (GKS) in the treatment of cystic brain metastasis. ⋯ The results of this study support the use of a multiple stereotactic approach in cases of multiple and cystic brain metastasis.

Case ReportsProgressive peritumoral edema defining the optic fibers and resulting in reversible visual loss.
Hemangioblastomas are frequently associated with peritumoral edema caused by extravasation of plasma ultrafiltrate through permeable neoplastic vessels. The authors report the clinical and imaging findings in a 62-year-old man with von Hippel-Lindau disease who presented with rapid (within 24 hours) loss of color vision and nearcomplete loss of left eye vision (acuity too poor to test). Serial MR imaging demonstrated a stable vascular tumor in the medioinferior aspect of the left optic nerve, associated with progressive edema extending from the nerve through to the bilateral optic radiations. ⋯ Postoperatively, the patient recovered color vision and had improvement in visual acuity (20/320). Serial imaging in this unique case captured the progressive extravasation of peritumoral edema that tracked and defined the parallel white matter tracts of first- and second-order neurons of the optic system, causing vision loss. Tumor resection led to resolution of the edema and improvement in visual function.

Comparative StudyRole of galectin-1 in migration and invasion of human glioblastoma multiforme cell lines.
Galectin-1 is highly expressed in motile cell lines. The authors investigated whether galectin-1 actually modulates the migration and invasion of human glioblastoma multiforme (GBM) cell lines, and whether its expression with respect to invasion and prognosis is attributable to certain glioma subgroups. ⋯ Diffuse gliomas demonstrated higher expression levels than pilocytic astrocytoma in the Western blot. Galectin-1 appears to modulate migration and invasion in human glioma cell lines and may play a role in tumor progression and invasiveness in human gliomas.
