Historical ArticleThe history of Bellevue Neurosurgery: a legacy of learning, discovery, and service.
The authors present a historical overview of NYU-Bellevue Neurosurgery, highlighting key events and influential faculty. Bellevue Hospital, the first public hospital in the US, was established in 1736 and has grown via its affiliation with New York University (now NYU Langone Health) from 1898 to the present. ⋯ NYU-Bellevue Neurosurgery began as a department in 1951 under Dr. Thomas Hoen and has since fostered notable faculty and graduates while contributing to the development of clinical neuroscience.

In vitro evaluation of flow diverter performance using a human fibrinogen-based flow model.
Fibrin deposition represents a key step in aneurysm occlusion, promoting endothelization of implants and connective tissue organization as part of the aneurysm-healing mechanism. In this study, the authors introduce a novel in vitro testing platform for flow diverters based on human fibrinogen. ⋯ Rates of fibrin deposition varied widely across different configurations and additive conditions in this novel in vitro model system. Fibrin accumulation started at the aneurysm inflow zone where flow velocity and shear stress were the highest. The primary factors influencing fibrin deposition included flow velocities, shear stress, and the addition of thrombin at a physiological concentration. Further research is needed to test the clinical utility of fibrinogen-based models for patient-specific aneurysms.

Flow capacity of a superficial temporal artery as a donor in a consecutive series of 100 patients with superficial temporal artery-middle cerebral artery bypass.
A superficial temporal artery-middle cerebral artery (STA-MCA) bypass is classically considered a low-flow bypass. It is known that the flow in the flow augmentation STA-MCA bypass is influenced by flow demand of the revascularized territory and can reach significantly higher values. The authors report their intraoperative flow measurement data in a consecutive series of 100 STA-MCA bypasses performed at their institution. Moreover, in a subanalysis, they show the postoperative bypass flow measured with quantitative MR angiography (qMRA) noninvasive optimal vessel analysis (NOVA). ⋯ Using intraoperative and postoperative quantitative flow measurements of the STA, the data confirm that the flow in the flow augmentation STA-MCA bypass is influenced by the flow demand of the revascularized territory and can reach high values if needed. Moreover, the significant flow increase in the postoperative flow measurement using qMRA-NOVA demonstrates that the bypass can increase its flow over time.

Our voice is powerful and important. ⋯ All contributors were invited to weave their insights regarding what truly matters into a broad, thought-provoking intellectual and spiritual tapestry. Universally resonant themes such as empathy, innovation, resilience, leadership, value, trust and equity framed the cooperative dialogues, emphasizing our shared humanity and the core values uniting us-despite our differences. The objectives for the 2024 Annual Meeting were ambitious: to consider as a professional community themes of utmost importance to our professional and personal lives; to catalyze a profound reevaluation of our collective objectives; to envision an expanded common mission; and to inspire visionary leaders to collaborate on creating lasting value-both for the patients who are the principal focus of our shared devotion, and for society writ large.

Observational StudyLong-term survivors in 976 supratentorial glioblastoma, IDH-wildtype patients.
Glioblastoma, isocitrate dehydrogenase (IDH)-wildtype is the most aggressive glioma with poor outcomes. The authors explored survival rates and factors associated with long-term survival in patients harboring a glioblastoma, IDH-wildtype. ⋯ Five-year overall survival in patients with glioblastoma, IDH-wildtype is extremely low. Predictors of a longer survival are mostly treatment factors, emphasizing the importance of a complete oncological treatment plan, when achievable. Glioblastoma, IDH-wildtype 5-year survivors could be screened for actionable targets in case of recurrence.
