Prospective clinical-radiographic study. ⋯ Changes in thoracic kyphosis on fulcrum bending due to natural coupling of deformities are directed towards "self-normalization." There is no difference in the sagittal plane deformity correction with the use of hook-rod system or pedicle screw-rod constructs. This can be used as a guideline for exact preoperative rod contouring to reduce the stress on the bone-implant interface and the rate of postoperative failures. The findings also suggest that it is not how big or strong the implants are, but rather the natural curve behavior will at least partially determine the final sagittal outcome.

Systematic review. ⋯ Malposition is the most commonly reported complication of thoracic pedicle screw placement, at a rate of 15.7% per screw inserted with postoperative computed tomography scans. The use of pedicle screws in the thoracic spine for the treatment of pediatric deformity has been reported to be safe despite the high rate of patients with malpositioned screws (11%). Major complications, such as neurologic or vascular injury, were almost never reported in this literature review of case series. Cases reports on the other hand have started to identify such complications.

Case report. ⋯ Severe complications, mainly associated with the disturbance in healing of the retropharyngeal wall, were observed in all 3 patients. A preventive method, such as primary repair of the retropharyngeal wall using muscular/musculocutaneous flaps, should be considered for patients undergoing resection through a transoral approach after ion-beam irradiation.

Electrophysiological analysis using somatosensory-evoked potentials (SEPs) and behavioral assessment using Basso, Beattie, Bresnahan (BBB) scale were compared over time for graded Multicenter Animal Spinal Cord Injury Study (MASCIS) contusion spinal cord injury (SCI). ⋯ SEP amplitudes and BBB scores decrease corresponding to increase in injury severity, however, these show different temporal patterns of recovery. These results demonstrate the utility of SEPs in conjunction with BBB, to monitor therapeutic interventions in SCI research.
