Variability of somatosensory cortical evoked potentials during spinal surgery. Effects of anesthetic technique and high-pass digital filtering.
The effects of anesthetic technique (nitrous oxide or propofol) and high-pass digital filtering on within-patient variability of posterior tibial nerve somatosensory cortical evoked potentials (PTN-SCEP) were compared prospectively in two groups of 20 patients undergoing spinal surgery. Average P1N1 amplitude was significantly higher and P1N1 amplitude variability lower during propofol/alfentanil anesthesia than during nitrous oxide/alfentanil anesthesia. ⋯ This study shows the importance of maintaining the highest possible PTN-SCEP amplitudes during spinal surgery. Propofol/opioid anesthesia may be an alternative anesthetic technique to nitrous oxide/opioid anesthesia during spinal cord function monitoring.

Epidural morphine for pain relief after lumbar laminectomy.
A prospective randomized trial was conducted to evaluate the effectiveness of epidural morphine for pain relief after lumbar laminectomy. Thirty-three male patients were studied in two groups. At the end of surgery, Group 1 patients (15) received 2 mg morphine in 5 ml saline through an epidural catheter. ⋯ There was no respiratory or cardiovascular depression detected in patients in either group. Nine patients in Group 1 and five patients in Group 2 had transient postoperative urinary retention that required catheterization. Only one patient in Group 1 had mild pruritus and three patients in Group 2 had nausea.

Chymopapain for the treatment of intervertebral disc herniation. A preliminary report of a double-blind study.
Sixty patients with unilateral sciatica resulting from lumbar intervertebral disc herniation were entered into a double-blind study. All patients had failed to respond to conservative management, and the diagnosis had been confirmed by metrizamide myelography. Following a randomized schedule, 30 patients had intradiscal injections of chymopapain and 30 patients had intradiscal injections of an equivalent volume of normal saline solution. ⋯ Six weeks after the injection, 73% of patients treated with chymopapain and 37% of patients treated with saline solution considered that the treatment was successful. The success rate at six months was 80% for patients treated with chymopapain and 57% for saline-injected patients. From the preliminary results of this study, chymopapain is recommended for the treatment of sciatica from intervertebral disc herniation when conservative measures have failed and before disc fragment excision is considered.
