Specific trunk and general exercise elicit similar changes in anticipatory postural adjustments in patients with chronic low back pain: a randomized controlled trial.
A randomized controlled trial. ⋯ SEG elicited significant reductions in self-rated disability and pain, whereas similar between-group changes in trunk muscle onsets were observed. The motor control adaptation seems to reflect a strategy of improved coordination between the trunk muscles with the unilateral shoulder movement. Trunk muscle onsets during rapid limb movement do not seem to be a valid mechanism of action for specific trunk exercise rehabilitation programs.

Who should have surgery for spinal stenosis? Treatment effect predictors in SPORT.
Combined prospective randomized controlled trial and observational cohort study of spinal stenosis (SpS) with an as-treated analysis. ⋯ With the exception of smokers, patients who met strict inclusion criteria improved more with surgery than with nonoperative treatment, regardless of other specific characteristics. However, TE varied significantly across certain subgroups, and these data can be used to individualize shared decision making discussions about likely outcomes. Smoking cessation should be considered before surgery for SpS.

The effect of early initiation of rehabilitation after lumbar spinal fusion: a randomized clinical study.
A multicenter randomized clinical trial including 82 patients. ⋯ Early start of rehabilitation (6 wk vs. 12 wk) after lumbar spinal fusion resulted in inferior outcomes. The improvements in the 12-week group were 4 times better than that in the 6-week group, indicating that the start-up time of rehabilitation is an important contributing factor for the overall outcome.

Prospective, randomized, controlled study. ⋯ In this prospective, randomized trial of a pedicle drilling device that uses electrical conductivity differentiation at the tip for assessing bone versus soft tissue, the number of fluoro shots was reduced by 30% compared with a standard drilling probe while maintaining a 97.5% accurate, safe screw placement.

Prospective randomized trial. ⋯ Although microbial sealant may be a useful addition to a multimodal approach to minimize surgical site infection, there is currently insufficient evidence as to whether the use of microbial sealants reduces the risk of surgical site infection in patients undergoing scoliosis surgery.
