Large distal anterior cerebral artery aneurysm treated with resection and interposition graft: case report.
Distal anterior cerebral artery (DACA) aneurysms are rare, representing only 2% to 6.7% of all intracranial aneurysms. Most of them are small. Large and giant aneurysms are even rarer in this location. Only 26 giant pericallosal (PC) aneurysms have been reported thus far. Various surgical techniques have been used to treat these aneurysms, including direct aneurysm neck clipping, aneurysm trapping, proximal occlusion of the anterior cerebral artery, or a combination of clipping with coiling or a bypass procedure. The report presents an unusual case of a complex DACA aneurysm managed by resection and interposition arterial graft. ⋯ Surgical treatment of a large DACA aneurysm may be difficult due to a complex neck and the involvement of the branch vessels. Resection and interposition grafting and A3-A3 or A4-A4 anastomoses are treatment options for such patients.

To review our experience with the surgical management of craniovertebral junction malformations, focusing on the selection of surgical approach, management of the associated Chiari malformation, and postoperative instability. ⋯ Successful decompression of the abnormal craniovertebral junction requires extensive preoperative evaluation, appropriate tailoring of the operative approach, and an adequate learning curve. Transmaxillary approaches are useful adjuncts to the transoral approach in patients with severe basilar invagination or in cases of limited jaw mobility. Anterior decompression has been proven effective in relieving obstruction of the subarachnoid space at the foramen magnum in most patients with associated Chiari malformation.

The purpose of this study was to delineate the anatomy of the precentral cerebellar vein, superior vermian vein, and internal occipital vein using reconstructions of computed tomographic and magnetic resonance imaging scans with navigation software. These data were compared with previous anatomic and angiographic findings to show the resolution and accuracy of the system. ⋯ This study illustrates the possibility of depicting the small vessels draining into the pineal region venous complex using 3-dimensional neuronavigation with an accuracy comparable to that of digital subtraction angiography. This tool provides important information for both surgical planning and intraoperative orientation.

VarioGuide (BrainLAB AG, Feldkirchen, Germany) is a new system for frameless image-guided stereotaxy. In the present study, we aimed to assess target point accuracy in a laboratory setting and the clinical feasibility of the system. ⋯ Concluding from the phantom measurements with ideal image-object registration, assumed spherical lesions with a volume of 0.524 cm can be biopsied with 100% target localization. Early clinical data revealed VarioGuide to be safe and accurate for lesions of 0.2 cm3 and larger. Thereby, the system seems feasible for the biopsy of most intracranial lesions.

The planning of retrosigmoid craniotomies often relies on anatomic landmarks on the posterolateral surface of the cranium, such as the asterion. However, the location of the asterion is not fixed with respect to the underlying transverse-sigmoid sinus complex. We introduce a simple procedure that uses 3-dimensional (3D) computed tomographic imaging to project the transverse-sigmoid sinus complex onto the external surface of the cranium. ⋯ Retrosigmoid craniotomy standardized according to anatomic landmarks raises the risk for sinus injury because of variations in their location. We offer our 3D computed tomographic imaging-based procedure as a useful device for the planning of safer retrosigmoid craniotomies.
