Clinical TrialEffects of the knee-chest position on cardiac index and propofol requirements during bispectral index (BIS)-guided spine surgery.
The knee-chest (KC) position is often used for spine surgery. It is considered to promote significant changes in venous return and cardiac output. However, the magnitude of these changes and their consequences on intraoperative haemodynamics and anaesthetic requirements remain to be determined. The goal of the present study was to determine the changes in cardiac index and propofol requirements of patients undergoing spine surgery in the KC position. ⋯ Placing surgical patients in the KC position during BIS guided anaesthesia was associated with marked decrease in cardiac index and propofol requirements. These results suggest that monitoring intraoperative cardiac index via an oesophageal Doppler and depth of anaesthesia with the BIS may be useful in patients undergoing spine surgery in the KC position.

Case Reports[Cranial subdural haematoma after spinal anaesthesia treated by blood patch].
The authors report a case of subdural haematoma after spinal anaesthesia. A 36-year-old woman underwent phlebectomy under spinal anaesthesia. ⋯ On the forth day, she was given a blood-patch, which improved rapidly the patient. Recovery was complete.

[Evaluation of NT-proBNP as an early biological marker of cardiac dysfunction in septic shock].
To evaluate the NT-proBNP as a biological diagnosis marker of the myocardial dysfunction in septic shock. ⋯ The NT-proBNP can help to detect the cardiogenic share sometimes implied in the haemodynamic failure of the septic shock. However, because of the influence of the renal insufficiency and the respiratory, cardiologic and hepatic comorbidities on its secretion, its use cannot be recommended for patients in septic shock.

[Use of Eschmann's gum elastic bougie in difficult orotracheal intubation, an audit over eight years of clinical practice].
The objective was to check on the effectiveness of difficult orotracheal intubation by Eschmann's gum elastic bougie in our institution. ⋯ Our study confirms the figures of publications, particularly the anglo-saxon ones and proves the interest of the Eschmann's gum elastic bougie in orotracheal intubations and this in first intention.

Comparative Study[Humidification assessment of four heat and moisture exchanger filters according to ISO 9360: 2000 standard].
This work consisted of the assessment of humidification parameters and flow resistance for different heat and moisture exchanger filters (HMEF) used in intensive care unit. Four electrostatic HMEF were assessed: Hygrobac S (Tyco); Humidvent compact S (Teleflex); Hygrovent S/HME (Medisize-Dräger); Clear-Therm+HMEF (Intersurgical). ⋯ This test showed differences between the HMEFs for both humidification and resistance parameters. When compared to the new version of the standards, HMEFs demonstrated their reliability. However, evolution of humidification and flow resistance characteristics over 24h showed a structural degradation of HMEFs, limiting their use over a longer period.
