Comparative Study Clinical Trial[High-flow nasal oxygen for severe hypoxemia after cardiac surgery].
High-flow nasal oxygen (Optiflow™) is validated in paediatric intensive care but not in adults' patients for severe hypoxemia. The aim of this study was to evaluate this oxygen system delivery in adults' patients for postoperative hypoxemia after cardiac surgery. ⋯ These results give good arguments for an improvement in gas exchange and better tolerance of high-flow nasal oxygen (Optiflow™) versus classical high-flow oxygen face mask in postoperative cardiac patients. These results must be confirmed by a randomised study with a larger population.

[Efficacy, safety and cost of sedation with sevoflurane in intensive care unit].
To study efficacy, systemic and cerebral haemodynamic, and cost of sedation with sevoflurane after midazolam failure. ⋯ Sevoflurane is an effective and safe alternative to midazolam in ICU patients associated with a moderate increase in costs.

Case Reports[Gas embolism: another story related to a central venous catheter withdraw].
Paradoxical gas embolism is rare but life threatening complication of central venous access. In this observation, we report a paradoxical gas embolism accompanied by an acute respiratory failure and a coma following a jugular central venous access withdrawal in a liver transplanted patient. Following this observation, we discuss the prevention of gas embolism accidents and the hyperbaric oxygen therapy for its management.

Case Reports[A rare cause of cardiogenic shock treated by extracorporeal life support: catecholamine secreting paraganglioma].
Phaeochromocytoma and paraganglioma are chromaffine tumours secreting catecholamines. They are usually revealed by a paroxystic hypertensive crisis associated with headaches, palpitation and sweats. We reported a case of a young patient presenting a state of life threatening cardiogenic shock as unusual revelation of this tumour, requiring the implementation of an extracorporeal life support until myocardial recovery.
