Randomized Controlled Trial Comparative Study
Comparison of analgesic effect of tramadol alone and a combination of tramadol and paracetamol in day-care laparoscopic surgery.
To compare the analgesic efficacy of tramadol alone (1.5 mg kg(-1)) with a tramadol (1 mg kg(-1)) and paracetamol combination in day-care laparoscopic patients. ⋯ We conclude that reducing the dose of tramadol to 1 mg kg(-1) and combining it with paracetamol 1 g orally decreased the incidence of side effects of tramadol without reducing analgesic efficacy.

To analyse the precision of transpulmonary thermodilution from the PiCCO technique (Pulsion Medical System, Munich, Germany) in everyday intensive care practice in order to ascertain the minimum number of measurements necessary for scientific precision. ⋯ Calculating the mean of two good-quality transpulmonary thermodilution measurements is equivalent to the other averaging techniques (three to five measurements) for the cardiac index and global end-diastolic volume index. Any further repeated measurements may be unnecessary and may contribute to volume overloading.

The analysis of perioperative cardiac arrest and mortality remains a potentially valuable method to improve clinical outcome. This survey evaluated the incidence and causes of perioperative cardiac arrests and mortality in a paediatric surgical population over 5 years. ⋯ Major risk factors for cardiac arrests were age under 1 year, poor physical status and emergency surgery. Respiratory and cardiovascular-related events accounted for most of all anaesthesia-related cardiac arrests. High ASA-PS was the leading cause of perioperative mortality.

Cardiac complications are a major cause of perioperative morbidity and mortality. These are caused by either myocardial ischaemia or acute coronary thrombosis. ⋯ This assessment allows measures to be taken that aim to reduce such risks. The present review summarizes the current state of knowledge on the preoperative assessment of the cardiac patient scheduled for noncardiac surgery.
