Infectious diseases of the musculoskeletal system are in trauma surgery sometimes the symptoms that most severely impair patients and are a challenge to surgeons, hospitals and the economy of the public healthcare system. The systemic capillary leak syndrome (SCLS) is a rare idiopathic syndrome that can provoke a fulminant life-threatening progress even if the primary infection is trivial. In addition to a clearly arranged description of the SCLS we report on an affected patient who developed a necrotizing myositis of the right thigh due to an unstable scar.

There is an accumulation of blast injuries on New Year's Eve due to fireworks. During an attempt to manufacture a firework the substances being mixed in a mortar exploded. ⋯ This case report shows the importance of expertise in hand surgery in the treatment of such severe injuries. A toe-to-hand transplantation represents an additional treatment option.

[Circular soft tissue defect after prolonged application of a strangulating compression stocking].
Wearing a compression stocking over a longer period of time can lead to deep skin and soft tissue defects. This article presents a case of a circular necrosis with an exposed tendon of the tibialis anterior muscle and the Achilles tendon. ⋯ Due to the short operating and anesthesia times, this flap is a good option for lower leg reconstruction, particularly in multimorbid patients. This case raises awareness of the importance of adequate patient training before commencing compression therapy.

[Acute rupture of the pectoralis major muscle at the musculotendinous junction : Case report of a rare injury and literature review].
Rupture of the pectoralis major muscle (PMR) is a rare injury that occurs primarily during strength training. The case of a 31-year-old professional basketball player who suffered a rupture of the pectoralis major muscle (PM) at the myotendinous junction while performing bench presses is presented. ⋯ The patient returned to playing basketball pain-free with full range of motion 3 months following refixation of the PM. This article discusses the distinct characteristics of this injury in the context of the current literature and the operative approach is presented in detail.

Numerous predisposing factors are known for patellar dislocations but the extent to which these or the trauma cause the dislocation is often unclear. ⋯ The predisposition rate in infantile patellar dislocations is high; however, falls are always significant accident events as well as medial direct impact. Torsional trauma is also a significant causative factor, unless high-grade trochlear dysplasia is present, whereas trivial traumas are not.
