Pancreas transplantation offers the possibility of preventing the development and progression of diabetic lesions by adequate control of hydrocarbon metabolism. Moreover, the diabetic patient is freed from dietary and physical restrictions, as well as from insulin treatment. However, this is achieved at the expense of an immunosuppression not always free of risk. ⋯ The introduction of cyclosporine has greatly improved the transplantation results, achieving a 53% actuarial survival of the graft per year. Out of the different technics used, the one that yields best results is the transplantation of the whole organ, deriving the exocrine secretion to the urinary bladder. This method also enables early diagnosis of graft rejection by monitoring urine amylase.

Review[Future perspectives of kidney transplant: towards a specific immunosuppression?].
The current status of immunosuppression in renal transplants is reviewed: non-specific (corticosteroids and cytostatics), lymphocyte-specific (polyclonal antilymphocyte globulin), T-specific (cyclosporine, antithymocyte polyclonal gamma globulin) with special attention to the OKT3 and monoclonal antibodies. Supported by the current experience of the use of PAN-T (OKT3) antiidiotypic monoclonal antibodies in the graft rejection treatment, a hypothesis is elaborated--feasible to demonstrate--to determine what would happen if anti-clonotypic monoclonal antibodies (the idiotypes of T lymphocytes) are used when looking for a specific immunosuppression. We conclude that the immunosuppression with anti-idiotypes, if it is confirmed, would be almost ideal.

Jaen is the province with the greatest number of patients with lepra. This study analyzes the prevalence of the disease in Jaen during 1984 and 1985. At that time, all the patients known to the health and consume provincial delegation of Jaén were studied. ⋯ The disease shows to have a highly significant and special predilection in gypsies and it is located in a significant geographical area in the province. Lepra did not show a rural distribution, however it prevails in middle size urban nuclei. Although all patients belonged to a low social class, the disease did not show to exhibit any association among the ecology and the income per inhabitant.

[Fructosamine and glycosylated hemoglobin as parameters of hyperglycemia].
We have determined the parameters fructosamine and A1 glycosylated hemoglobin, observing a good correlation between them. When analyzing both techniques, the determination of fructosamine proved to be more precise. Considering the significance of each of the tests, we conclude that these parameters cannot substitute for each other but are complementary and that the convenience of fructose determination can not invalidate the utility of determining the glycosylated hemoglobin.
