Randomized Controlled Trial Comparative Study Clinical Trial
Dissociation between haemodynamics and sympathetic activation during anaesthetic induction with desfluranes.
To compare the simultaneous haemodynamic effects, sympathetic activation and cardiac risks associated with desflurane used in a balanced technique, with those of isoflurane anaesthesia. ⋯ Desflurane differs from isoflurane in that sympathetic stimulation persisted despite blunting of potential hyperdynamic haemodynamic responses by narcotic and beta blockade. However, this sympathetic activation did not appear to increase cardiac risks.

Pharmacokinetics of doxacurium during normothermic and hypothermic cardiopulmonary bypass surgery.
To compare the pharmacokinetic behaviour of doxacurium in patients undergoing normothermic or hypothermic cardiopulmonary bypass (CPB) for coronary artery bypass graft surgery. ⋯ The elimination rate of doxacurium during normothermic CPB is faster than that in hypothermic CPB.

Midazolam improves postoperative epidural analgesia with continuous infusion of local anaesthetics.
Midazolam has been shown to have an analgesic effect by single shot epidural administration. In this study, the effect of midazolam on continuous epidural infusion of bupivacaine was investigated to find a better combination for postoperative analgesia. ⋯ Adding midazolam to a continuous epidural infusion of bupivacaine provides better analgesia, amnesia and sedation than bupivacaine alone without side effects in patients undergoing laparotomy.

Case Reports
Deafness and prolonged neuromuscular blockade following single-dose peritoneal neomycin irrigation.
A case of deafness is reported in a patient undergoing bilateral nephrectomy after single irrigation of the peritoneal cavity with a neomycin containing solution for surgical prophylaxis. ⋯ Antibiotic irrigation solutions containing neomycin may produce unwanted adverse systemic effects of deafness and prolonged neuromuscular blockade, even with only brief application within the peritoneal cavity. Concurrent end-stage renal disease, visceral inflammation from chronic dialysis and laparotomy, and the concomitant use of other medications may have contributed to these unwanted effects.
